摘 要:目的分析糖尿病足Wagner分级与葡萄糖范围内时间(TIR)的相关性。方法回顾性分析2020年1月至2021年12月在江西省人民医院内分泌科住院治疗的糖尿病足患者共155例,根据糖尿病足Wagner分级法分为3组:0级90例、1~2级28例和3~4级37例,比较3组一般资料、生化指标以及动态葡萄糖监测指标并进行Spearman相关分析。结果3组年龄、病程、收缩压、舒张压、TC、TG、LDL-C、HDL-C、BUN、CRE、FA、Hcy、TIR、TAR、MBG、SDBG、MAGE、CV比较,差异具有统计学意义(P<0.05)。Spearman相关分析结果显示:糖尿病足Wagner分级与LDL-C、TAR、SDBG、MAGE、CV呈正相关(r=0.536~0.767);与性别、舒张压、TC、HDL-C、TIR与呈负相关(r=-0.158~-0.759)。结论糖尿病足Wagner分级与TIR、TBR负相关,与TAR正相关,糖尿病足Wagner分级越高,血糖控制越差。LDL-C、SDBG、MAGE、CV是糖尿病足的危险因素,女性、舒张压、TC、HDL-C是糖尿病足的保护因素。Objective To analyze the correlation between Wagner grading of diabetic foot and time in glucose range(TIR).Methods A total of 155 patients with diabetic foot hospitalized in the Department of Endocrinology of Jiangxi Provincial People s Hospital from January 2020 to December 2021 were retrospectively analyzed.According to Wagner classification,90 patients were classified into grade 0,28 patients were classified into grade 1~2 and 37 patients were classified into grade 3~4.The general data,biochemical indexes and dynamic blood glucose monitoring indexes of the three groups were compared and Spearman analysis was performed.Results There were statistically significant differences in age,course of disease,systolic blood pressure,diastolic blood pressure,TC,TG,LDL-C,HDL-C,BUN,CRE,FA,Hcy,TIR,TAR,MBG,SDBG,MAGE and CV among the three groups(P<0.05).Spearman correlation analysis showed that Wagner grading was positively correlated with LDL-C,TAR,SDBG,MAGE and CV(r=0.536~0.767).It was negatively correlated with sex,diastolic blood pressure,TC,HDL-C and TIR(r=-0.158~-0.759).Conclusion Wagner grading of diabetic foot was negatively correlated with TIR and TBR and positively correlated with TAR,and blood glucose control was worse.LDL-c,SDBG,MAGE and CV are risk factors for diabetic foot,while female,diastolic blood pressure,TC and HDL-C are protective factors for diabetic foot.
