摘 要:目的分析不同厂家线性免疫印迹法(LIA)试剂盒以及LIA与ELISA在检测可疑系统性红斑狼疮(SLE)患者血清样本特异性自身抗体的一致性。方法分析LIA试剂盒A(LIA-A)和试剂盒B(LIA-B)检测313例可疑SLE患者的血清样本抗双链DNA抗体(抗dsDNA抗体)、抗Sm抗体和抗核小体抗体的一致性。313例患者血清样本中,随机选取60例患者的血清样本,进一步采用ELISA检测抗dsDNA抗体、抗Sm抗体、抗核小体抗体,比较LIA与ELISA检测结果的一致性。结果LIA-A与LIA-B检测抗dsDNA、抗Sm抗体、抗核小体抗体结果不一致分别为24、14、39例(kappa=0.670、0.727、0.373,P<0.01)。LIA-A与ELISA检测抗dsDNA抗体、抗Sm抗体、抗核小体抗体结果不一致分别为16、5、9例(kappa=0.385、0.833、0.695,P<0.05或P<0.01)。LIA-B与ELISA检测抗dsDNA抗体、抗Sm抗体、抗核小体抗体结果不一致分别为15、11、28例(kappa=0.414、0.633、0.187,P<0.05或P<0.01)。结论对于抗Sm抗体,不同检测方法和试剂之间一致性一般或较好。对于抗dsDNA抗体,LIA-A与ELISA检测一致性较差。对于抗核小体抗体,LIA-A与LIA-B及LIA-B与ELISA检测一致性较差。Objective To analyze the consistency of linear immunoblotting(LIA)kits from different manufacturers and two methods of LIA and ELISA in detecting the specific autoantibodies in serum samples from the patients with suspected systemic lupus erythematosus(SLE).Methods The consistency of LIA kit A(LIA-A)and kit B(LIA-B)in detecting anti-double-stranded DNA antibody(anti-dsDNA antibody),anti-Sm antibody,and anti-nucleosome antibody in serum samples from 313 suspected SLE patients was analyzed.From 313 serum samples,60 serum samples were randomly selected and tested for anti-dsDNA antibody,anti-Sm antibody and anti-nucleosome antibody using LIA kit and ELISA.The consistency of the detection results of LIA and ELISA was compared.ResultsThe numbers with inconsistent results of anti-dsDNA antibody,anti-Sm antibody and anti-nucleosome antibody detected by LIA-A and LIA-B were 24,14,39 cases,respectively(kappa=0.670,0.727,0.373,P<0.01).The numbers with inconsistent results of anti-dsDNA antibody,anti-Sm antibody and anti-nucleosome antibody detected by LIA-A and ELISA were 16,5,9 cases,respectively(kappa=0.385,0.833,0.695,P<0.05 or P<0.01).The numbers with inconsistent results of anti-dsDNA antibody,anti-Sm antibody and anti-nucleosome antibody detected by LIA-B and ELISA were 15,11,28 cases,respectively(kappa=0.414,0.633,0.187,P<0.05 or P<0.01).Conclusion For detecting anti-Sm antibody,the consistency of different detection methods and reagents is moderate or good.For detecting anti-dsDNA antibody,the consistency of LIA-A and ELISA detection is poor.For detecting anti-nucleosome antibody,the consistency of LIA-A and LIA-B detection and consistency of LIA-B and ELISA detection are poor.
