摘　　要：目的探索行为习惯逆转疗法(HRT)联合心理护理对老年皮肤瘙痒症患者搔抓行为、心理状态、生活质量及睡眠质量的影响。方法采用回顾性调查研究法,以滕州市中心人民医院2019年6月至2020年6月收治的126例老年皮肤瘙痒症患者为研究对象,应用电脑进行随机数字表法分组,将患者分为参照组与试验组,每组63例,给予参照组以常规护理干预,试验组在此基础上行HRT联合心理护理干预,对比两组患者的搔抓行为、心理状态及生活质量。结果干预后,试验组患者的每日搔抓次数、新增皮肤破损例数、病情中重度例数及皮肤感染例数均显著小于参照组,差异有统计学意义(P<0.05);试验组患者的焦虑自评量表(SAS)及抑郁自评量表(SDS)评分、皮肤病生活质量指数(DLQI)、匹兹堡睡眠质量指数(PSQI)量表评分显著均低于参照组,差异有统计学意义(均P<0.05)。结论针对老年皮肤瘙痒症患者,应用HRT联合心理护理干预能够有效改善患者的搔抓行为及睡眠质量,缓解其不良心理,提升其生活质量。Objective To explore the effects of habit reversal therapy(HRT)combined with psychological nursing on scratching behavior,mental state,quality of life and sleep quality in senile patients with pruritus.Methods A retrospective study was conducted on 126 patients with senile pruritus treated in this hospital from June 2019 to June 2020.The patients were divided into the control group and the experimental group with 63 cases in each group.The control group was given routine nursing intervention,and the experimental group was given HRT combined with psychological nursing intervention on this basis.Scratching behavior,mental state and quality of life were compared between the two groups.Results After intervention,the number of daily scratching,new cases of skin damage,cases of moderate and severe disease and cases of skin infection in experimental group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Self-rating Anxiety Scale(SAS)and Self-rating Depresssion Scale(SDS)scores,Dermatology Life Quality Index(DLQI)scores and Pittsburgh Sleep Quality Index(PSQI)scale scores of experimental group were significantly lower than those of the control group,and the differences were statistically significant(P<0.05).Conclusions For senile patients with pruritus skin,the application of HRT combined with psychological nursing intervention can effectively improve the scratching behavior and sleep quality of patients,relieve their bad psychology,improve their quality of life.
