摘　　要：患者男,44岁,汉族,2个月前无明显诱因左手无名指伸侧出现一直径约3 mm大小淡红色丘疹,表面光滑,无瘙痒疼痛不适。双上肢、颜面部、躯干、下肢逐步出现上述皮疹。患者1年前因头面部聚合性痤疮口服异维A酸治疗6个月。皮肤科情况:颜面部、躯干、四肢可见数个散在分布淡红色丘疹,呈绿豆至黄豆大小,表面光滑,部分中央可见脐凹,表面无鳞屑、破溃,触之质软,与基底部粘连紧密,活动度差,与周围组织境界清楚,无压痛。甘油三脂2.71 mmo/L↑。皮损组织病理示:表皮萎缩变薄,真皮层可见泡沫样组织细胞弥漫或呈结节状浸润,可见Touton巨细胞,夹杂数量不一的淋巴细胞。诊断:发疹性黄瘤病。给予调脂及二氧化碳点阵激光治疗后皮疹未见进展。A 44-year-old male,Han nationality,presented with a light red papule with diameter of 3 mm on the extended side of left ring finger without obvious triggers for 2 months.The surface was smooth without itching,pain and discomfort.Since then,the above rash gradually appeared on both upper limbs,face,trunk and lower limbs.One year ago,the patient was treated with isotretinoin orally for 6 months due to acne vulgaris on the head and face.Physical examination showed that light red papules varied in size in face,trunk and limbs,smooth surface,soft touch,umbilical fovea in the center,no scales and ulcers,adheres closely to the base,poor mobility,clear boundary,without tenderness.Triglyceride 2.71 mmo/L↑.Histopathological examination showed that the epidermis atrophy and thinning.The dermis can be seen as diffuse or nodular infiltration of foam like tissue cells.Touton giant cells was found,mixed with varying numbers of lymphocytes.Diagnosis:eruptive xanthomatosis.The rash did not progress after lipid-lowering and carbon dioxide fractional laser treatment.
