慢性阻塞性肺疾病患者脑血流灌注与健康人群的差异性研究

摘　　要：目的比较分析慢性阻塞性肺疾病(COPD)患者脑血流灌注与健康人群的差异。方法选取COPD患者(COPD组)和与之年龄、性别匹配的健康对照(HC)(HC组)各20例。所有受试者的脑血流灌注成像均使用动脉自旋标记(ASL)成像,并由设备以同等条件重建出脑血流量(CBF)图,由2名放射科副主任医师目视检查图像质量,保证图像质量符合分析要求;使用MATLAB、SPM、DAPABI等软件,对COPD组和HC组的CBF图基于体素分析,结果使用FDR校正,P<0.05为差异有统计学意义。结果COPD组全脑平均血流灌注水平与HC组无差异,右侧苍白球、左侧岛叶、右侧岛叶、右侧额下回、右侧顶叶、右侧额中回6个脑区血流灌注水平与HC组有差异,对应t值分别为:4.72、-4.23、-5.05、-3.78、-5.14、-5.16,所有t值对应的P<0.05,t值为正表示COPD组局部脑区血流灌注较HC组增高,t值为负表示较HC组血流灌注减低。结论COPD组整体脑血流灌注水平与HC组无差异,右侧苍白球血流灌注较HC组增高,左侧岛叶、右侧岛叶、右侧额下回、右侧顶叶、右侧额中回血流灌注水平较HC组减低。Objective To compare and analyze the difference of cerebral blood perfusion between patients with chronic obstructive pulmonary disease(COPD)and healthy people.Methods Twenty patients with COPD patients(COPD group)and 20 healthy control(HC)(HC group)matched with age and sex were selected.Cerebral blood perfusion imaging of all subjects was performed using arterial spin labeling(ASL)imaging,and cerebral blood flow(CBF)map was reconstructed by the equipment under the same conditions.The image quality was visually checked by two deputy chief radiologists to ensure that the image quality met the analysis requirements.MATLAB,SPM,DPABI and other software were used to analyze the CBF maps of COPD group and HC group based on voxel analysis.The results were corrected by FDR,P<0.05 was considered statistically significant.Results There was no difference in the average cerebral blood perfusion level between COPD group and HC group,but there were differences in the cerebral blood perfusion level in the right globus pallidus,left insula,right insula,right inferior frontal gyrus,right parietal lobe and right middle frontal gyrus between COPD group and HC group.The corresponding t values were as follows:4.72,-4.23,-5.05,-3.78,-5.14,-5.16,all t values corresponding to P<0.05.A positive t value indicates that the local cerebral blood perfusion of COPD group was higher than that of HC group,while a negative t value indicates that the blood perfusion of COPD group was lower than that of HC group.Conclusion There is no difference in the overall cerebral blood perfusion level between COPD group and HC group.Compared with HC group,the blood perfusion level of right globus pallidus is higher,and the blood perfusion level of left insula,right insula,right inferior frontal gyrus,right parietal lobe,and right middle frontal gyrus is lower.
