儿童小脑蚓部和脑干MR解剖测量的研究

摘　　要：目的研究儿童小脑蚓部和脑干MR解剖测量参数,以提高诊断小脑蚓部和/或脑桥发育不良的准确性。方法回顾性选取接受MR检查的0~12岁儿童600例作为研究对象,记录小脑蚓部和脑干参考值进行统计学分析。儿童测量的MR参考数据为小脑蚓部和脑干解剖测量的蚓部高度(H-V)、蚓部前后径(APD-V)、中脑桥连接处的前后径(APD-MP)、中脑桥的前后直径(APD-P)值。结果Pearson相关性分析显示,性别与不同参数(H-V、APD-V、APD-MP、APD-P)无相关性(P>0.05);年龄与不同参数(H-V、APD-V、APD-MP、APD-P)有相关性(P<0.05)。H-V、APD-V、APD-MP、APD-P的组内相关系数(ICC)[95%置信区间(CI)]与一致性检验均>0.90,表明观察者内部对每个参数的一致性都很好,ICC在0.906~0.977之间,观察者之间的一致性从良好到极好。观察者间的一致性很好,ICC范围从APD-P的0.876到H-V的0.936。结论儿童测量的MR参考数据与年龄增长变化有显著相关性,并且在小脑异常相关疾病中得到进一步验证。Objective To study the anatomic measurement parameters of the cerebellar vermis and brain stem on MR in children,to improve the accuracy of diagnosing cerebellar vermis and/or pontine dysplasia.Methods A total of 600 children aged 0-12 years who underwent MR examination were retrospectively selected as the research objects,the cerebellar vermis and brain stem reference values were recorded for statistical analysis.The MR reference data measured of children were H-V,APD-V,APD-MP,APD-P diameter values measured by cerebellar vermis and brain stem anatomic.Results Pearson correlation analysis showed that there was no correlation between sex and different parameters(H-V,APD-V,APD-MP,APD-P)(P>0.05);the age was correlation with different parameters(H-V,APD-V,APD-MP,APD-P)(P<0.05).The intraclass correlation coefficient(ICC)[95%confidence interval(CI)]confidence and consistency tests for H-V,APD-V,APD-MP,APD-P,all greater than 0.90,showed that the consistency within the observer for each parameter was better,with the ICC between 0.906 and 0.977,and the consistency between observers ranging from good to excellent.The consistency between observers was good,with ICC ranging from 0.876 in APD-P to 0.936 in H-V.Conclusion The MR reference data of children measure are significantly correlated with changes in age and should be further validate in cerebellar abnormalities-related diseases.
