摘　　要：目的分析非接触式全视野镜联合微创玻璃体手术对增生期糖尿病性视网膜病变(PDR)患者中心视力和黄斑厚度的影响。方法回顾性系列病例研究。选取2021年1~8月我院接收的220例(220只眼)PDR患者,按照治疗方法的不同分为研究组和对照组,各组110例(110只眼)。对照组采用23G微创玻璃体视网膜切除系统治疗,研究组采用非接触式全视野镜联合23G微创玻璃体视网膜切除系统治疗。对比两组手术时间和并发症发生情况,比较两组手术前、手术后1、3、6个月最佳矫正视力(BCVA)和黄斑中心视网膜厚度(CMT)。结果研究组手术时间较对照组低(P<0.05)。重复测量数据的方差分析显示,组别和时间对BCVA均有影响,且两者存在交互作用。组别和时间对CMT均有影响,但两者不存在交互作用。研究组并发症发生率低于对照组(P<0.05)。结论PDR患者行非接触式全视野镜联合微创玻璃体切除术有利于提高视力,减轻黄斑水肿,并且该术式可缩短手术时间,安全性高,值得推广。Objective To analyze the effects of non-contact total field of vision combined with minimally invasive vitreous on central visual acuity and macular thickness in proliferative diabetic retinopathy(PDR)patients.Methods A retrospective case series study.220 patients(220 eyes)with PDR in our hospital from January 2021 to August 2021 were selected and divided into study group and control group according to different treatment methods,110 cases(110 eyes)in each group.The control group was treated with 23G minimally invasive vitreoretinal resection system,and the study group was treated with non-contact full field lens combined with 23G minimally invasive vitreoretinal resection system.Operation time and complications were compared between the two groups,and the best corrected visual acuity and macular central retinal thickness(CMT)were compared between the two groups before the operation,at 1 month,3 months and 6 months after the operation.Results The operation time of the study group was lower than that of the control group(P<0.05).Anova of repeated measurements showed that the group and time had an effect on best corrected visual acuity,and there is interaction between them.The group and time had an effect on CMT,but there was no interaction between them.The complication rate in the study group was lower than that in the control group(P<0.05).Conclusions Non-contact total field of vision combined with minimally invasive vitrectomy for PDR patients is beneficial to improve visual acuity and reduce macular edema,and this method can shorten the operation time,has high safety,and is worth promoting.
