全方位护理干预对多发肋骨骨折合并肺挫裂伤患者术后疼痛程度及睡眠质量的影响

摘　　要：目的:探讨在多发肋骨骨折合并肺挫裂伤患者护理中采取全方位护理干预的临床效果,观察对患者术后疼痛以及睡眠质量的影响。方法:选取2020年6月至2021年6月福建省厦门市同安区中医医院收治的多发肋骨骨折合并肺挫裂伤患者66例作为研究对象,按照随机数字表法分为对照组和观察组,每组33例。对照组给予常规护理干预,观察组给予全方位护理干预。采用匹兹堡睡眠质量指数(PSQI)比较2组患者睡眠质量的差异,视觉模拟评分法(VAS)比较2组患者的疼痛感受,比较2组患者术后康复指标、治疗依从性及并发症发生率。结果:治疗后,观察组患者治疗依从性显著高于对照组,观察组VAS、PSQI评分显著低于对照组,观察组疼痛持续时间、胸腔引流管留置时间、住院时间均短于对照组,观察组并发症发生率显著低于对照组,差异有统计学意义(P<0.05)。结论:在多发肋骨骨折合并肺挫裂伤患者的护理中实施全方位护理干预,可有效缓解患者的疼痛感受,提高治疗依从性,促进病情好转,提高睡眠质量,建议推广应用。Objective:To investigate the clinical effect of comprehensive nursing intervention in the nursing of patients with multiple rib fracture complicated with lung contusion and laceration,and observe the effect on postoperative pain and sleep quality of patients.Methods:A total of 66 patients with multiple rib fracture combined with lung contusion and laceration admitted to Tong′an District Hospital of Traditional Chinese Medicine in Xiamen,Fujian Province from June 2020 to June 2021 were selected as research objects,and divided into observation group and control group according to the random number table method,with 33 cases in each group.The control group was given routine nursing intervention,and the observation group was given comprehensive nursing intervention.Pittsburgh Sleep Quality Index(PSQI)was used to compare the difference of sleep quality between the 2 groups,and Visual Analogue Scale(VAS)was used to compare the pain perception between the 2 groups.Postoperative rehabilitation indexes,treatment compliance and incidence of complications were compared as well.Results:After treatment,the treatment compliance of the observation group was significantly higher than the control group,VAS and PSQI scores of the observation group were significantly lower than that of the control group,the pain duration,thoracic drainage indentation time and hospital stay of the observation group were shorter than the control group,the complication rate of the observation group was significantly lower than the control group,and the difference between the 2 groups was statistically significant(P s<0.05).Conclusion:The comprehensive nursing intervention in the nursing of patients with multiple rib fractures combined with lung contusion and laceration can effectively relieve the pain of patients,improve the treatment compliance,promote the improvement of the condition,improve the quality of sleep,it is recommended to popularize the application.
