手术内固定治疗踝关节骨折合并下胫腓联合韧带损伤的效果

摘　　要：目的:探讨手术内固定治疗踝关节骨折合并下胫腓联合韧带损伤的效果。方法:本次研究将前瞻性选取2020年12月-2023年1月在毕节市中医医院治疗的84例踝关节骨折合并下胫腓联合韧带损伤患者为受试对象,以随机数字表法将其分研究组与对照组,各42例。对照组应用关节复位后保守治疗,研究组应用手术内固定治疗,观察并比较两组患者的疗效、骨折愈合效果、治疗前后视觉模拟评分法(visual analogue scale,VAS)评分与踝关节功能Ogilvie-Harris踝关节评分,同时记录不良反应发生情况。结果:研究组总有效率较对照组高,差异有统计学意义(P<0.05)。研究组骨折愈合优良率明显优于对照组,差异有统计学意义(P<0.05)。治疗后两组VAS评分、Ogilvie-Harris踝关节评分较治疗前均显著改善,差异均有统计学意义(P<0.05);治疗后研究组的VAS评分显著低于对照组,差异有统计学意义(P<0.05),Ogilvie-Harris踝关节评分显著高于对照组,差异有统计学意义(P<0.05)。研究组的不良反应发生率较对照组更低(P<0.05)。结论:应用手术内固定治疗踝关节骨折合并下胫腓联合韧带损伤效果确切,该治疗方式可以更好地改善患者踝关节损伤,促进骨折愈合效率,缓解疼痛,不良反应少,安全性可靠。Objective:To explore the effectiveness of surgical internal fixation in the treatment of ankle joint fractures combined with lower tibiofibular ligament injury.Method:This study prospectively selected 84 patients with ankle fracture combined with lower tibiofibular ligament injury treated in Bijie Hospital of Traditional Chinese Medicine from December 2020 to January 2023 as the test subjects.The patients were divided into study group and control group by random number table method,42 cases in each group.The control group received conservative treatment after joint reduction,and the study group received surgical internal fixation.The therapeutic effect,fracture healing effect,visual analogue scale(VAS)score and ankle joint function Ogilvie-Harris ankle joint score before and after treatment were observed and compared between the two groups,while the occurrence of adverse reactions was recorded.Result:The total effective rate of the study group was higher than that of the control group,the difference was statistically significant(P<0.05).The excellent and good rate of fracture healing in the study group was significantly higher than that in the control group,the difference was statistically significant(P<0.05).After treatment,the VAS score and Ogilvie-Harris ankle joint score of the two groups were significantly improved compared to before treatment,the differences were statistically significant(P<0.05);the VAS score of the study group was significantly lower than that of the control group,the difference was statistically significant(P<0.05),while the Ogilvie-Harris ankle joint score was significantly higher than that of the control group,the difference was statistically significant(P<0.05).The incidence of adverse reactions in the study group was significantly lower than that in the control group(P<0.05).Conclusion:The application of surgical internal fixation for the treatment of ankle joint fracture combined with lower tibiofibular ligament injury has a definite effect.This treatment method can better improve t
