摘 要:目的:探究及分析特殊膳食调理对妊娠期糖尿病患者血糖变异性及氧化损伤的影响。方法:将2021年8月-2022年8月佳木斯市妇幼保健院诊治的100例妊娠期糖尿病患者根据随机数字表法分为对照组和观察组,各50例。对照组进行常规妊娠及糖尿病干预,观察组则在对照组的基础上进行特殊膳食调理。比较两组血糖控制效果、干预前后的氧化损伤指标[活性氧(ROS)、晚期氧化蛋白产物(AOPP)、一氧化氮(NO)]、常规血糖指标[空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)]、血糖变异性相关指标[餐后血糖波动幅度(PPGE)、血糖标准差(SDBG)、平均血糖波动幅度(MAGE)及空腹血糖变异系数(CV-FPG)]。结果:观察组的血糖控制效果显著优于对照组,差异有统计学意义(P<0.05)。干预前两组的氧化损伤指标、常规血糖指标及血糖变异性相关指标比较,差异均无统计学意义(P>0.05);干预4、8周后,两组ROS、AOPP、常规血糖指标及血糖变异性相关指标均显著降低,且观察组均显著低于对照组,NO显著升高,且观察组显著高于对照组,差异均有统计学意义(P<0.05)。结论:特殊膳食调理可有效控制妊娠期糖尿病患者的血糖,对血糖变异性及氧化损伤也有较好的改善作用。Objective:To investigate and analyze the effect of special dietary conditioning on blood glucose variability and oxidative damage in patients with gestational diabetes mellitus.Method:A total of 100 patients with gestational diabetes mellitus diagnosed and treated in Maternal and Child Health Center of Jiamusi City from August 2021 to August 2022 were divided into control group and observation group according to random number table method,with 50 cases in each group.The control group received routine pregnancy and diabetes intervention,and the observation group received special dietary conditioning on the basis of the control group.The blood glucose control effect,before and after intervention of oxidative damage indexes[reactive oxygen species(ROS),advanced oxidation protein products(AOPP),nitric oxide(NO)],routine blood glucose indexes[fasting plasma glucose(FPG),2 hours postprandial blood glucose(2 h PG),glycosylated hemoglobin(HbA1c)]and blood glucose variability related indexes[postprandial glucose excursions(PPGE),standard deviation of blood glucose(SDBG),mean amplitude of glycemic excursion(MAGE),coefficient of variation-fasting plasma glucose(CV-FPG)]between the two groups were compared.Result:The blood glucose control effect of the observation group was significantly better than that of the control group,the difference was statistically significant(P<0.05).Before intervention,there were no significant differences in oxidative damage indexes,routine blood glucose indexes and blood glucose variability related indexes between the two groups(P>0.05).After 4 and 8 weeks of intervention,ROS,AOPP,routine blood glucose indexes and blood glucose variability related indexes were significantly decreased in both groups,and those in the observation group were significantly lower than those in the control group,NO was significantly increased,and that in the observation group was significantly higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion:The special dietary con
