手部烧伤后瘢痕挛缩畸形采用全厚皮片移植术的效果观察

摘　　要：目的:探讨手部烧伤后瘢痕挛缩畸形采用全厚皮片移植术的效果。方法:将中国人民解放军联勤保障部队第九〇七医院烧伤整形科2020年1月—2022年1月收治的70例手部烧伤后瘢痕挛缩畸形患者以随机数字表法分为参照组和试验组。试验组(n=35)以全厚皮片植皮术进行治疗,参照组(n=35)于瘢痕切除后进行皮瓣修复术治疗。观察两组临床疗效、手部关节功能、并发症发生情况、创面愈合时间及手部美观满意度评分。结果:试验组治疗总有效率相较于参照组明显更高,差异有统计学意义(P<0.05)。治疗后,两组总主动活动度(TAM)均较治疗前有明显提高,且试验组TAM相较于参照组明显更高,差异均有统计学意义(P<0.05)。试验组并发症总发生率相较于参照组明显更低,差异有统计学意义(P<0.05)。试验组创面愈合时间相较于参照组明显更短,手部美观满意度评分相较于参照组明显更高,差异均有统计学意义(P<0.05)。结论:在手部烧伤后瘢痕挛缩畸形患者的临床治疗中,全厚皮片移植术具有良好的治疗效果,不仅有利于促进患者手部关节功能恢复,降低各种并发症发生率,而且还可以进一步帮助患者缩短创面愈合时间,提高患者手部美观满意度。Objective:To investigate the effect of full-thickness skin graft transplantation for scar contracture deformity after hand burn.Method:A total of 70 patients with scar contracture deformity after hand burn admitted to the Burn and Plastic Department of NO.907 Hospital,Joint Logistics Support Force of the Chinese People's Liberation Army from January 2020 to January 2022 were divided into the reference group and the experimental group according to random number table method.The experimental group(n=35)was treated with full-thickness skin graft transplantation,and the reference group(n=35)was treated with skin flap repair after cicatrectomy.The clinical efficacy,hand joint function,incidence of complications,wound healing time and the scores of hand aesthetic satisfaction between the two groups were observed.Result:The total effective rate in the experimental group was significantly higher than that in the reference group,the difference was statistically significant(P<0.05).After treatment,the total action motion(TAM)of the two groups were significantly higher than those before treatment,and the TAM of the experimental group was significantly higher than that of the reference group,the differences were statistically significant(P<0.05).The total incidence of complications in the experimental group was significantly lower than that in the reference group,the difference was statistically significant(P<0.05).The wound healing time of the experimental group was significantly shorter than that of the reference group,and the the scores of hand aesthetic satisfaction was significantly higher than that of the reference group,the differences were statistically significant(P<0.05).Conclusion:In the clinical treatment of patients with scar contracture deformity after hand burn,full-thickness skin graft transplantation has good therapeutic effect,which is not only conducive to promoting the recovery of patients'hand joint function and reducing the incidence of various complications,but also can further help patients shorten wo
