冰冻血浆在颅脑损伤患者中的应用效果及对血栓弹力图参数的影响

摘　　要：目的:探究冰冻血浆在颅脑损伤患者中的应用效果及对血栓弹力图参数的影响。方法:将2019年6月—2022年10月宜春新建医院的100例颅脑损伤根据随机数字表法分为对照组50例和观察组50例。对照组进行常规颅脑损伤治疗,观察组则在对照组的基础上加用冰冻血浆进行治疗。比较两组颅脑损伤治疗总有效率、治疗前后的神经损伤指标[血清神经元特异性烯醇化酶(NSE)、神经丝轻链蛋白(NF-L)及胶质纤维酸性蛋白(GFAP)]、凝血指标[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)及纤维蛋白原(FIB)]及血栓弹力图参数[纤维蛋白原功能(α角)、最大振幅(MA值)、凝血启动阶段凝血时间(R值)及凝血时间(K值)]。结果:观察组的颅脑损伤治疗总有效率显著高于对照组,差异有统计学意义(P<0.05),治疗前两组的神经损伤指标、凝血指标及血栓弹力图参数比较,差异均无统计学意义(P>0.05),治疗后24、72 h观察组的神经损伤指标、PT、APTT及K值均显著低于对照组,FIB及α角、MA值、R值均显著高于对照组,差异均有统计学意义(P<0.05)。结论:冰冻血浆在颅脑损伤患者中的应用效果较好,对于神经损伤的改善作用较好,且可有效改善凝血及血栓弹力图参数。Objective:To investigate the application effect of frozen plasma in the patients with craniocerebral injury and influence for thromboelastogram parameters.Method:A total of 100 patients with craniocerebral injury in Yichun Xinjian Hospital from June 2019 to October 2022 were divided into control group 50 cases and observation group 50 cases by the random number table method.The control group were treated with routine treatment for craniocerebral injury,the observation group were treated with frozen plasma on the treatment of control group.The total effective rates of craniocerebral injury treatment,nerve injury indexes[serum neuron-specific enolase(NSE),neuronal light chain protein(NF-L)and glial fibrillary acidic protein(GFAP)],coagulation indexes[prothrombin time(PT),activated partial thromboplastin time(APTT)and fibrinogen(FIB)]and thromboelastogram parameters[fibrinogen function(αangle),platelet function(MA value),reaction time(R value)and clot formation rate(K value)]before and after the treatment were compared between two groups.Result:The total effective rate of craniocerebral injury treatment of observation group was significantly higher than that of control group,the difference was statistically significant(P<0.05).The nerve injury indexes,coagulation indexes and thromboelastogram parameters of two groups before the treatment were compared,the differences were not statistically significant(P>0.05).The nerve injury indexes,PT,APTT and K value of observation group at 24,72 h after the treatment were significantly lower than those of control group,and the FIB,αangle,MA value and R value were significantly higher than those of control group,the differences were statistically significant(P<0.05).Conclusion:The application effect of frozen plasma in the patients with craniocerebral injury is better,its improvement role for the nerve injury is better,and it can effectively improve the coagulation and thromboelastogram parameters.
