摘　　要：目的:探究超声引导下穿刺冲洗联合中药治疗非哺乳期乳腺炎的临床效果。方法:将2018年1月-2022年8月启东市中医院收治的80例非哺乳期乳腺炎患者根据随机数字表法分为对照组和观察组,各40例。对照组进行中药治疗,观察组则进行超声引导下穿刺冲洗联合中药治疗。比较两组总有效率,脓肿持续时间,治疗前后的红、肿、热积分、疼痛程度[视觉模拟评分法(VAS)]、生活质量[世界卫生组织生存质量测定量表(WHOQOL-BREF)]及炎症因子水平[C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)及白介素-1β(IL-1β)]。结果:观察组治疗总有效率显著高于对照组,脓肿持续时间显著短于对照组(P<0.05)。治疗前两组红、肿、热积分、VAS评分、WHOQOL-BREF评分及炎症因子水平比较,差异均无统计学意义(P>0.05);治疗1、2周后,观察组红、肿、热积分、VAS评分及炎症因子水平均显著低于对照组,WHOQOL-BREF各维度评分均显著高于对照组(P<0.05)。结论:超声引导下穿刺冲洗联合中药治疗非哺乳期乳腺炎的临床效果较好,可有效改善患者的不适感及炎症状态。Objective:To investigate the clinical effect of ultrasound-guided puncture and irrigation combined with traditional Chinese medicine on non-lactation mastitis.Method:A total of 80 patients with non-lactation mastitis admitted to Qidong Hospital of TCM from January 2018 to August 2022 were divided into control group and observation group according to random number table method,with 40 cases in each group.The control group was treated with traditional Chinese medicine,and the observation group was treated with ultrasound-guided puncture and irrigation combined with traditional Chinese medicine.The total effective rate,duration of abscess,red swelling heat integrals,degree of pain[visual analogue scale(VAS)],quality of life[WHO Quality of Life-BREF(WHOQOL-BREF)]and the levels of inflammatory factors[C reactive protein(CRP),tumor necrosis factor-α(TNF-α)and interleukin-1β(IL-1β)]before and after treatment were compared between the two groups.Result:The total effective rate in the observation group was significantly higher than that in the control group,and the duration of abscess was significantly shorter than that in the control group(P<0.05).Before treatment,there were no significant differences in red swelling heat integrals,VAS scores,WHOQOL-BREF scores and the levels of inflammatory factors between the two groups(P>0.05).After one and two weeks of treatment,the red swelling heat integrals,VAS scores and the levels of inflammatory factors in the observation group were significantly lower than those in the control group,and the scores of each dimension of WHOQOL-BREF were significantly higher than those in the control group(P<0.05).Conclusion:Ultrasound-guided puncture and irrigation combined with traditional Chinese medicine has a good clinical effect in the treatment of non-lactation mastitis,which can effectively improve the discomfort and inflammatory state of patients.
