自拟通腑降糖方治疗2型糖尿病胃肠自主神经病变的疗效分析

摘　　要：目的观察自拟通腑降糖方治疗2型糖尿病胃肠自主神经病变(type 2 diabetic gastrointestinal autonomic neuropathy,T2DM-GAN)的疗效。方法回顾性选取2017年9月—2019年6月桐城市中医院收治的107例T2DM-GAN患者的临床资料,根据不同治疗方法分为观察组(n=69)和对照组(n=38),两组均在常规治疗糖尿病的基础上对症治疗。观察组在此基础上给予中药通腑降糖方加减治疗。比较两组临床疗效,血糖水平,中医临床症状积分及不良反应发生情况。结果观察组总有效率高于对照组,差异有统计学意义(P<0.05)。两组治疗后的FPG和HbA1c水平低于治疗前;治疗后,观察组的HbA1c水平低于对照组,差异有统计学意义(P<0.05);治疗后两组FPG比较,差异无统计学意义(P>0.05)。两组治疗后的中医证候积分低于治疗前;治疗后,观察组餐后上腹饱胀感、反酸嗳气、胃脘疼痛、恶心呕吐积分低于对照组,差异有统计学意义(P<0.05)。两组治疗前后均无不良反应发生。结论自拟通腑降糖方有助于T2DM-GAN患者的血糖稳定,改善患者胃肠道症状。Objective The efficacy of self-mimic hypoglycemic prescription in the treatment of type 2 diabetic gastro-intestinal autonomic neuropathy(T2DM-GAN)was observed.Methods A retrospective study was performed for the clinical data of 107 patients with T2DM-GAN admitted to Tongcheng Hospital of Traditional Chinese Medicine from September 2017 to June 2019,and divided into observation group(n=69)and control group(n=38)according to differ-ent treatments,and both groups were symptomatic on the basis of conventional treatment of diabetes.On this basis,the observation group was given the addition and subtraction of the traditional Chinese medicine tongfu hypoglycemic formula.The clinical efficacy,blood glucose level,TCM clinical symptom score and occurrence of adverse reactions of the two groups were compared.Results The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).The levels of FPG and HbA1c in the two groups after treatment were lower than those before treatment,and the levels of HbA1c in the observation group were lower than those in the control group after treatment,the difference was statistically significant(P<0.05),and the dif-ference between the two groups after treatment was not statistically significant(P>0.05).The TCM syndrome scores of the two groups after treatment were lower than those before treatment.After treatment,the postprandial epigastric full-ness,acid belching,stomach pain,nausea and vomiting scores in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).There were no adverse reactions before and af-ter treatment in either group.Conclusion Self-mimic hypoglycemic formula can help stabilize blood glucose and im-prove gastrointestinal symptoms in patients with T2DM-GAN.
