摘　　要：目的探讨缩减约束方案在降低PICU患儿非计划性拔管和约束部位皮肤损伤发生的应用效果。方法选择2020年1—7月入住某三级甲等儿童专科医院PICU收治的281例重症患儿作为研究对象,按照组间基线资料可比的原则分为观察组143例和对照组138例。对照组采用常规身体约束护理流程,观察组采用缩减约束方案。比较两组患儿身体约束率、非计划性拔管发生率及约束部位皮肤损伤发生率。结果干预后,观察组非计划性拔管率、皮肤损伤发生率均低于对照组,但差异无统计学意义(P>0.05);干预后,观察组身体约束率低于对照组,差异有统计学意义(P<0.05)。结论缩减约束方案可降低PICU患儿的身体约束率,具有降低非计划性拔管率及皮肤损伤发生率的潜在趋势。Objective To explore the application effect of reduced restraint program in reducing unplanned extubation and skin injury at restraint sites in PICU children.Methods 281 critically ill children admitted to PICU of a Class III Grade A children's hospital from January to July 2020 were selected as the research objects.According to the principle of comparability of baseline data between groups,they were divided into observation group(143 cases)and control group(138 cases).The control group adopted the routine physical restraint nursing process,and the observation group adopted the reduced restraint program.The physical restraint rate,the incidence of unplanned extubation and the incidence of skin injury at the restraint site were compared between the two groups.Results After intervention,the unplanned extubation rate and the incidence of skin injury in the observation group were lower than the control group,the difference was not statistically significant(P>0.05).After intervention,the physical restraint rate of the observation growp was lower than the control group,the difference was statistically significant(P<0.05).Conclusion The reduced restraint program can reduce the physical restraint rate of PICU children,and has a potential trend to reduce the unplanned extubation rate and the incidence of skin injury.
