针药并用治疗阴虚血燥型原发性干燥综合征26例临床观察

摘　　要：目的:研究针药并用治疗阴虚血燥型原发性干燥综合征患者的临床疗效。方法:将52例原发性干燥综合征患者随机分为治疗组和对照组,每组26例。两组均给予硫酸羟氯喹片口服,每次0.2 g,每日2次。治疗组在对照组治疗基础上联合针刺治疗,每周2次。两组疗程均为12周。观察对比两组临床疗效及治疗前后中医证候积分等。澳洲抗风湿联盟干燥综合征患者报告指数(EULAR SS Patient Reported Index,ESSPRI)、泪液流率、唾液流率、红细胞沉降率(Erythrocyte Sedimentation Rate,ESR)、C反应蛋白(C-reactive Protein,CRP)、类风湿因子(Rheumatoid Factor,RF),治疗后评价临床疗效。结果:治疗组总有效率显著优于对照组(P<0.05)。治疗后,两组中医证候积分、ESSPRI评分均降低,泪液流率、唾液流率均升高,ESR、CRP、RF均降低,且治疗组的各项指标改善显著优于对照组(P<0.05)。结论:针刺联合硫酸羟氯喹治疗原发性干燥综合征患者疗效确切,安全可靠。此法能明显改善口干、眼干症状,降低ESSPRI评分,升高泪液流率、唾液流率,降低ESR、CRP、RF,调节免疫功能。Objective:To study the clinical effect of acupuncture and medicine on patients with primary Sjogren’s syndrome(SS)of Yin(阴)deficiency and blood dryness type.Methods:A total of 52 patients with primary SS were randomly divided into the treatment group and the control group,with 26 cases in each group.Both groups were given hydroxychloroquine sulfate tablets orally,0.2 g each time,twice a day.The treatment group was treated with acupuncture on the basis of the control group,twice a week.The course of treatment was 12 weeks.The clinical efficacy and TCM syndrome score,EULAR SS patient reported index(ESSPRI),tear flow rate,saliva flow rate,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),rheumatoid factor(RF)before and after treatment in the two groups were observed.Results:The total efficiency in the treatment group was higher than that in the control group(P<0.05).After treatment,TCM syndrome score and ESSPRI score were decreased.Tear flow rate and saliva flow rate were increased.ESR,CRP and RF were decreased in both groups.The improvement of indexes in the treatment group was significantly better than that in the control group(P<0.05).Conclusion:Acupuncture combined with hydroxychloroquine sulfate is effective,safe and reliable in the treatment of patients with primary SS.It can significantly improve the symptoms of dry mouth and dry eyes,reduce ESSPRI score,increase tear flow rate and saliva flow rate,reduce ESR,CRP and RF,and regulate immune function.
