马文辉运用甘苦泻心汤治疗狐惑病临床经验

摘　　要：狐惑病首载于东汉著名医学家张机(字仲景)所著《金匮要略》。此病是一种慢性自身免疫性疾病,常见症状为口、眼、肛(或外阴)溃烂,并有神志反应的综合征,相当于西医的白塞病(Behcet Disease,BD)。狐惑病迁延日久,对患者的精神、生活造成极大困扰。目前临床研究表明,中医药在提高狐惑病的总治愈率、降低治疗后复发率等方面均优于单纯的西药治疗。山西省中医药大学附属医院马文辉教授根据狐惑病的发病机制、病理特点及临床表现等,发现寒热错杂证兼夹湿热蕴结证是狐惑病长期、反复发作的主要因素,并且是狐惑病发作最常见的证型。马文辉教授根据多年临床经验总结,在甘草泻心汤的基础上加苦参、土茯苓治疗狐惑病疗效更佳。文章结合案例,介绍马文辉教授三部六病验方甘苦泻心汤治疗狐惑病的经验,以期为临床提供参考。Huhuo disease(狐惑)was first recorded in the Eastern Han Dynasty famous physician Zhang Ji’s Jigui Yaolue(《金匮要略》).This disease is a chronic autoimmune disease.The common symptoms are ulceration of mouth,eyes,anus(or vulva),and a syndrome of consciousness reaction,which is equivalent to behcet disease(BD)in western medicine.The prolonged illness has caused great distress to the patient’s spirit and life.Current clinical studies have shown that TCM medicine in improving the total cure rate,reduce the recurrence rate after treatment are better than western medicine.According to the pathogenesis,pathological characteristics and clinical manifestations of Huhuo disease,Professor MA Wenhui of the Affiliated Hospital of Shanxi University of Traditional Chinese Medicine found that the syndrome of intermingled heat and cold combined with damp-heat accumulation is the main factor of long-term recurrent Huhuo disease,and is the most common syndrome type of Huhuo disease.Based on many years of clinical experience,Professor MA Wenhui found that on the basis of the Gancao Xiexin decoction(甘草泻心汤),the curative effect of adding Kushen(Radix Sophorae Fiavescentis)and Tu Fuling(Rhizoma Smilacis Glabrae)was better in the treatment.Based on the case,this paper introduces Professor MA Wenhui’s experience in treating Huhuo disease with the Ganku Xiexin decoction(甘苦泻心汤),in order to provide reference for clinical practice.
