摘　　要：变应性血管炎是变态反应性疾病,较为难治。此病临床上极易误诊为过敏性紫癜、臁疮、湿疹等,因此对诊断尤为重要。本病常在紫癜的基础上伴有皮肤坏死,皮肤损伤多从足背沿内外踝呈线状向上蔓延,溃疡时伴有疼痛。该病病情较重时,常伴随系统性损害,累及周围神经、中枢神经及内脏等。山西中医药大学附属医院郑学军主任认为,本病多为青壮年,素体阳气偏盛,蕴热于内,外受风寒湿邪侵袭,营卫不和,寒湿入里化热,湿热蕴结,瘀滞气血,痹阻脉络而发病。郑老师总结出该病病情发展分为三个证型,即湿热下注型、血瘀湿阻型、气血两虚型。治疗上,应西医辨病与中医辨证相结合,内治外治相结合,以提高临床疗效。文章通过案例,介绍郑学军主任治疗变应性血管炎的成功经验,以期为临床参考。Allergic vasculitis is an allergic disease,which is difficult to treat.In clinic,this disease is easily misdiagnosed as allergic purpura,ecthyma,eczema,etc.,so it is particularly important for diagnosis.The disease is often accompanied by skin necrosis on the basis of purpura.The skin damage mostly spreads from the dorsum of the foot to the inner and outer ankles in a linear way.The ulcer is accompanied by pain.When the disease is severe,it is often accompanied by systemic damage,involving peripheral nerves,central nerves and internal organs.ZHENG Xuejun,the director of the Affiliated Hospital of Shanxi University of Traditional Chinese Medicine,believes that This disease is mostly young and middle-aged,the body Yang Qi(阳气)is prosperous,the heat is inside,the outside is attacked by wind and cold and dampness,the camp guard is discordant,the cold and humidity enter the heat inside,the humid heat is knotted,stagnant Qi and blood,and the veins are blocked.Teacher ZHENG concluded that the development of the disease is divided into three types,the Shire Xiazhu type(湿热下注型),the Xueyu Shizu type(血瘀湿阻型),and the Qixue Liangxu type(气血两虚型).In terms of treatment,we should combine disease differentiation of western medicine with syndrome differentiation of TCM medicine,and combine internal treatment with external treatment to improve the clinical efficacy.This article introduces the successful experience of Director ZHENG Xuejun in treating allergic vasculitis through cases,with a view to providing clinical reference.
