益肾通络方治疗无症状性特发性少弱精子症不育临床研究

摘　　要：目的:观察益肾通络方治疗无症状性特发性少弱精子症不育的临床疗效。方法:130例患者随机为对照组(65例,脱落5例)和治疗组(65例,脱落6例),对照组采用左卡尼汀口服溶液治疗,治疗组采用益肾通络方治疗。观察并比较两组患者治疗前及治疗4、8、12周后的精液体积、精子总数、精子浓度、前向运动精子百分率(PR)、精子总活力(PR+NP)、直线运动速度(VSL)、精子功能(精子尾部低渗膨胀率、精子顶体酶活性),精子DNA碎片指数(DFI)等情况,比较两组患者配偶妊娠率,评价临床疗效。结果:与本组治疗前比较,治疗12周,两组患者的精液体积、精子浓度、精子总数、PR、PR+NP、VSL、精子DFI、精子顶体酶活性、精子尾部低渗膨胀率均显著改善(P<0.05),且除精液体积外,治疗组均优于对照组(P<0.05);治疗组总有效率为83.05%(49/59),显著高于对照组的61.67%(37/60)(P<0.05);治疗组患者配偶妊娠率为16.95%(10/59),高于对照组的10.00%(6/60),但差异无统计学意义;整个临床研究过程中患者均未出现明显不良反应及一般安全性指标异常。结论:益肾通络方可提高无症状性特发性少弱精子症不育的生育能力,且具有较好的安全性。Objective:To observe the clinical efficacy of Yishen Tongluo Formula in treating asymptomatic idiopathic oligoasthenozoospermia and infertility.Methods:A total of 130 patients were randomly divided into the control group(65 cases,5 cases of detachment)and the treatment group(65 cases,6 cases of detachment).The control group was treated with levocarnitine oral solution,while the treatment group was treated with Yishen Tongluo Formula.Observe and compare the semen volume,total sperm count,sperm concentration,progressive(PR),total sperm motility(PR+NP),linear motion velocity(VSL),sperm function(sperm tail hypotonic expansion rate,sperm acrosome),enzyme activity and sperm DNA fragmentation index(DFI),etc of two groups of patients before treatment and after 4,8,and 12 weeks of treatment.Compare the pregnancy rate of spouses between two groups of patients and evaluate clinical efficacy.Results:Compared with before treatment,after 12 weeks of treatment,the semen volume,sperm concentration,total sperm count,PR,PR+NP,VSL,sperm DFI,sperm acrosin activity,and sperm tail hypotonic expansion rate of both groups of patients were significantly improved(P<0.05),and the treatment group was superior to the control group except for semen volume(P<0.05);The treatment group total effective rate was 83.05%(49/59),significantly higher than the control group’s 61.67%(37/60)(P<0.05);The pregnancy rate of spouses in the treatment group was 16.95%(10/59),higher than the 10.00%(6/60)of the control group,but the difference was not statistically significant.Throughout the clinical research process,no significant adverse reactions and safety were observed in the patients abnormal indicators.Conclusion:Yishen Tongluo Formula can improve the fertility of asymptomatic idiopathic oligoasthenozoospermia infertility and has good safety.
