针罐引经导气松解术治疗非特异性下腰痛临床观察

摘　　要：目的:比较针罐引经导气松解术与单纯针刺治疗非特异性下腰痛(NLBP)的临床疗效。方法:将70例患者随机分为对照组与试验组,每组35例。两组选穴相同,为腰部阿是穴、命门、腰阳关,双侧肾俞、大肠俞、委中、阳陵泉;对照组行单纯针刺治疗,每日1次,每周治疗5次,共治疗2周(10次);试验组行针罐引经导气松解术,隔日治疗1次,每周治疗3次,共治疗2周(6次)。分别于治疗前后观察两组患者的视觉模拟评分(VAS),腰椎活动度及E值,Oswestry功能障碍指数(ODI)、总分,中位频率(MF)值和均方根(RMS)值。结果:与本组治疗前比较,治疗后两组患者VAS、ODI总分及ODI均显著下降(P<0.01),腰椎活动度及E值均显著升高(P<0.01),且试验组优于对照组(P<0.01);治疗后两组患者MF值较治疗前呈下降趋势,RMS值较治疗前差异无统计学意义;治疗后两组患者MF值及RMS值差异无统计学意义。结论:针罐引经导气松解术可有效改善NLBP患者疼痛感觉、腰椎活动度及功能障碍,其改善作用优于对照组。Objective:To compare the clinical efficacy of acupuncture and cupping guiding meridians and qi relaxation and simple acupuncture in the treatment of nonspecific low back pain(NLBP).Methods:A total of 70 patients were randomly divided into a control group and an experimental group,with 35 patients in each group.The two groups selected the same acupoints:including Ashi acupoint,Mingmen(DU 4),and Yaoyangguan(DU 3)in the waist,as well as bilateral Shenshu(BL 23),Dachangshu(BL 25),Weizhong(BL 40),and Yanglingquan(GB 34);The control group received simple acupuncture treatment once a day and 5 times a week for a total of 2 weeks(10 times);The experimental group underwent acupuncture and cupping guiding meridians and qi relaxation,with treatment once every other day and three times a week for a total of 2 weeks(6 times).Before and after treatment,the visual analogue score(VAS),lumbar range of motion and E value,Oswestry dysfunction index(ODI)and total score,median frequency(MF)value,and root mean square(RMS)value of both groups of patients were observed.Results:Compared with before treatment in this group,after treatment,the VAS,total score of ODI,and ODI in both groups of patients significantly decreased(P<0.01),while lumbar range of motion and E value significantly increased(P<0.01),and the experimental group was superior to the control group(P<0.01);After treatment,the MF values of the two groups of patients showed a decreasing trend compared to before treatment,while the RMS values showed no statistically significant difference compared to before treatment;There was no statistically significant difference in MF and RMS values between the two groups of patients after treatment.Conclusion:The acupuncture and cupping guiding meridians and qi relaxation can effectively improve the pain sensation,lumbar range of motion and dysfunction of NLBP patients,and its improvement effect is better than those of the control group.
