摘 要:目的:设计新型眼科手术中可调节供氧及托盘系统,解决眼科局部麻醉手术患者吸氧管佩戴繁琐、配戴体验差及术中无菌单覆盖所致的憋热不适等问题,并评价其安全性及有效性。方法:新型眼科手术中可调节供氧及托盘系统由床体固定器、可调节托盘、可调节支撑杆、万向调节支撑氧气管、氧气输出接口和前后水平延伸支架组成。选取在医院行玻璃体切除治疗因增殖性糖尿病视网膜病变的67例患者,按照随机数表法将其分为观察组(33例)和对照组(34例),观察组术中使用新型眼科手术中可调节供氧及托盘系统,对照组术中使用传统手术托盘加胶布固定鼻吸氧管方式,对比两组患者入室连接氧气前(T0)、连接氧气后(T1)、无菌辅料单覆盖粘贴眼科贴膜后(T2)、开睑器撑开眼睑10min(T3)及手术结束后撤离无菌辅料单后(T4)5个时间点的血氧饱和度及心率、患者对手术满意度、术前准备时长及医生对手术满意度,评价两组托盘应用的安全性及有效性。结果:观察组患者对新型眼科手术中可调节供氧及托盘系统非常满意32例(占97%),医生对观察组患者手术非常满意31例(占94%),均明显高于对照组,其差异有统计学意义(x^(2)=17.27,x^(2)=59.84;P<0.05);观察组患者术前准备时间及术中不适调整时间分别为(8.0±3.0)s和(38.0±5.0)s,均明显短于对照组,其差异有统计学意义(t=-23.684,t=-5.211;P<0.05);两组患者的心率与血氧饱和度无统计学差异。结论:新型眼科可调节供氧及托盘系统安全有效,可明显缩短手术开台时间,提高连台效率,以及患者和医生的满意度。Objective:To design a novel adjustable oxygen supply and tray system in ophthalmic surgery to solve the problems of cumbersome wearing of oxygen inhalation tube,poor wearing experience and discomfort caused by sterile sheet covering in ophthalmic surgery,and to evaluate its safety and effectiveness.Methods:A novel adjustable oxygen supply and tray system for ophthalmic surgery was designed,which was composed of six components:bed fixator,adjustable tray,adjustable support rod,universal adjustable support oxygen tube,oxygen output interface,and frontback horizontal extension bracket.Sixty-seven patients who underwent vitrectomy for proliferative diabetic retinopathy in the hospital were selected and divided into observation group(33 cases)and control group(34 cases)according to the random number table method.The observation group used the newly designed adjustable oxygen supply and tray system in ophthalmic surgery,the control group used a traditional surgical tray.The blood oxygen saturation and heart rate,patient satisfaction,preoperative preparation time and surgeon satisfaction were compared between the two groups at five different time points.The safety and effectiveness of the tray applications in the two groups were evaluated.Results:The 32 patients in the observation group were satisfied with the new adjustable oxygen supply and tray system in ophthalmic surgery,and the surgeons were satisfied with the operation process of 31 patients in the observation group,which were significantly higher than those in the control group,the difference was statistically significant(x^(2)=17.27,x^(2)=59.84,P<0.05).The preoperative preparation time and intraoperative discomfort adjustment time of the observation group were(8.0±3.0)seconds and(38.0±5.0)seconds,respectively,which were significantly shorter than those of the control group,the differences were statistically significant(t=-23.684,t=-5.211,P<0.05).There was no statistical difference in heart rate and oxygen saturation between the two groups.Conclusion:The newly
