急性胆囊炎继发胆囊床肝脓肿的临床诊治(附4例报道)并文献复习

摘　　要：当急性胆囊炎炎症较重或持续时间较长时,胆囊周围可形成炎性液体积聚,当炎症侵及周围组织或器官或并发胆囊穿孔时可形成胆囊周围脓肿,当炎症向肝床方向侵犯,可突破胆囊壁全层,从而波及胆囊壁外,甚至侵犯至胆囊床肝组织,导致邻近肝组织的炎症,严重者可继发胆囊床肝脓肿。胆囊床肝脓肿为胆囊周围脓肿的一种少见类型,且临床处理棘手,保守治疗效果欠佳,手术治疗亦有相当难度。近2年收治例该类患者4例,现将临床诊治经过及文献复习汇报如下。When the inflammation of acute cholecystitis is severe or lasts for a long time,inflammatory fluid can be formed around the gallbladder to accumulate.And when the inflammation invades the surrounding tissues or organs or is complicated with gallbladder perforation,a peri-gallbladder abscess can be formed.When the inflammation invades the liver bed,it can break through the whole layer of the gallbladder wall,thus affecting the outside of the gallbladder wall,or even invading the liver tissue of the gallbladder bed,leading to inflammation in the adjacent liver tissue,and even secondary liver abscess in the gallbladder bed in severe cases.Gallbladder bed liver abscess is a rare type of peri-gallbladder abscess,and it is difficult to manage clinically,with unsatisfactory results of conservative treatment,and quite difficult to operate on.In the past two years,we have treated four patients of this type.The clinical diagnosis and treatment as well as literature review are reported below.
