针刺联合重复经颅磁刺激治疗帕金森病运动症状临床观察

摘　　要：目的:观察针刺联合重复经颅磁刺激对改善帕金森病患者运动功能的临床疗效。方法:纳入帕金森病患者40例,随机分为观察组和对照组,各20例。对照组予以SMA区rTMS 5Hz高频刺激2周,观察组予以SMA区rTMS 5 Hz高频刺激2周联合针刺四关穴2周。比较治疗前、治疗后20 min及治疗后1个月2组运动功能评分(UPDRSⅢ评分、震颤评分、强直评分、运动迟缓评分),以及2组神经电生理指标(RMT、MEP120、CMCT、CSP)。结果:2组治疗后20 min、治疗后1个月运动功能总评分及分项评分均较治疗前改善(P<0.05),且观察组改善更为明显(P<0.05)。2组治疗后20 min的运动功能总评分及分项评分均优于治疗后1个月的评分(P<0.05)。2组治疗后20 min、治疗后1个月神经电生理指标均较治疗前改善(P<0.05),且观察组改善更为明显(P<0.05)。2组治疗后20 min的神经电生理指标均优于治疗后1个月的指标(P<0.05)。结论:针刺联合重复经颅磁刺激能更好地改善运动功能及神经电生理结果,可将其应用于帕金森病患者,以期提高临床疗效。Objective:To explore whether acupuncture combined with repetitive transcranial magnetic stimulation(rTMS)can improve motor function and achieve better clinical efficacy in patients with Parkinson's disease.Methods:40 patients with Parkinson's disease were randomly divided into two groups.In the single treatment group,20 patients were stimulated with rTMS 5Hz for 2 weeks;In the combined treatment group,20 cases were treated with rTMS 5 Hz high-frequency stimulation in SMA area for 2 weeks,combined with acupuncture at Siguan point for 2 weeks.The motor function scores(UPDRSIII score,tremor score,ankylosis score,bradykinesia score)and electrophysiological data(RMT,MEP120,CMCT,CSP)of the two groups were collected before treatment,20 minutes after treatment and 1 month after treatment.Results:the total scores and sub scores of motor function in the two groups were improved 20 minutes and 1 month after treatment,but the improvement in the combined treatment group was more obvious(P<0.05).The total score and sub score of motor function at 20 minutes after treatment in both groups were better than those at 1 month after treatment(P<0.05).The electrophysiological data of the two groups at 20 min and 1 month after treatment were improved compared with those before treatment,but the improvement was more obvious in the combined treatment group(P<0.05).The electrophysiological data at 20 minutes after treatment in both groups were better than the scores at 1 month after treatment(P<0.05).Conclusion:acupuncture combined with repetitive transcranial magnetic stimulation can improve motor function and neuroelectrophysiological results better than single repetitive transcranial magnetic stimulation.Therefore,acupuncture combined with repetitive transcranial magnetic stimulation can be applied to patients with Parkinson's disease,in order to improve the clinical efficacy.
