摘  要:目的 探讨血清25-羟基维生素D[25(OH)D]的水平与老年性高血压及其脑卒中发生发展的相关性。方法 选择2020年12月至2021年5月在昆明市延安医院老年病科住院的年龄≥60岁高血压患者共236例作为研究对象,按其高血压程度,将病例分为2级高血压组(72例)和3级高血压组(164),选取同期健康体检者66例作为对照组。收集研究对象临床资料,留取研究对象血清检测25(OH)D及其他生化指标,应用统计学软件SPSS 26.0对数据进行统计学分析。结果2级高血压和3级高血压组与对照组相比较,2级高血压组的25(OH)D、TC、HDL-C、LDL-C与对照组比较水平降低(P<0.05),年龄、BMI、脉压差与对照组比较升高(P<0.05)。3级高血压组的25(OH)D、TC、HDL-C、LDL-C与对照组比较降低(P<0.05),年龄、BMI、脉压差与对照组比较升高(P<0.05)。与2级高血压相比较,3级高血压组的脉压差、血糖、TC、LDL-C差异均有统计学意义(P<0.05),且均比2级高血压组升高。高血压组分层分析后,单纯高血压组25(OH)D水平较高血压合并卒中组明显升高(P<0.001)。Pearson相关分析高血压患者血清25(OH)D与年龄、脉压差和GLU呈负相关,与身高和体重呈正相关。单因素Logistics回归分析变量因素收缩压、25(OH)D和血糖是高血压发生脑卒中的危险因素。结论 低水平的25(OH)D与老年高血压的发生、发展有相关性,25(OH)D降低可能是高血压发生发展和导致脑卒中的风险因子。Objective To investigate the correlation between serum 25-hydroxyvitamin D[25(OH)D]levels and hypertension in elderly population.Methods A total of 236 hypertensive patients of aged≥60 years hospitalized in the Department of Geriatrics,Yan’an Hospital,Kunming Medical University from December,2020 to May,2021 were recruited for the study.According to the degree of hypertension,these patients were divided into the hypertension grade 2 group(72 cases)and hypertension grade 3 group(164 cases),while 66 healthy subjects during the same period were selected as the control group.25(OH)D and its biochemical parameters were measured by Beckman Coulter AU5421 automatic biochemical analyzer.The relationship between the control group and hypertensions was analyzed by statistical software SPSS 26.0.Results Compared with the control group,25(OH)D,TC,HDL-C and LDL-C in the hypertension grade 2 group were lower(P<0.05),while age,BMI and SBP-DBP in the hypertension grade 2 group were higher(P<0.05).25(OH)D,TC,HDL-C and LDL-C in the hypertension grade 3 group were lower(P<0.05),while age,BMI and SBP-DBP were higher(P<0.05).Compared with grade 2 hypertension,SBP-DBP,blood glucose,TC and LDL-C in the hypertension grade 3 group were statistically different(P<0.05),and all were higher than that in the grade 2 hypertension group.After stratified analysis,the levels of 25(OH)D in the group with simple hypertension was significantly higher than that in the group with stroke of hypertension(P<0.001).Pearson correlation analysis showed that serum 25(OH)D level was negatively correlated with age,SBP-DBP and GLU,while positively correlated with height and weight.The variables of body weight,systolic blood pressure,TC and 25(OH)D were risk factors for hypertension and stroke.Conclusion Low level of 25(OH)D is correlated with the occurrence and development of hypertension in the elderly population.Decreased 25(OH)D may be a risk factor for the development of hypertension and stroke.
