摘  要:目的 研究宫颈脱落细胞中微小RNA-424(miR-424)和枯灵素2(CUL2)信使RNA(mRNA)表达水平对宫颈人乳头瘤病毒(HPV)16持续感染的预测价值。方法 选取2020年1月至2021年11月在邯郸市中心医院宫颈液基细胞学检查正常的259例宫颈HPV16感染者为研究对象,随访1年后复查,HPV16仍是阳性者归为HPV16持续感染组(136例),HPV16转为阴性者归为HPV16非持续感染组(123例)。对宫颈HPV16持续感染者在复查当日以组织病理学诊断结果进行分组:52例慢性宫颈炎症患者(宫颈炎症组),61例宫颈上皮内瘤变者(上皮瘤变组),23例宫颈鳞癌患者(宫颈鳞癌组)。采用荧光定量PCR法检测宫颈脱落细胞中miR-424和CUL2 mRNA表达水平,Pearson法分析HPV16持续感染者宫颈脱落细胞中miR-424和CUL2 mRNA的相关性,受试者工作特征曲线评价宫颈脱落细胞中miR-424和CUL2 mRNA表达水平对宫颈HPV16持续感染的预测价值。结果 与HPV16非持续感染组相比,HPV16持续感染组宫颈脱落细胞中miR-424表达水平较低,CUL2 mRNA表达水平较高(P均<0.001);宫颈炎症组、上皮瘤变组、宫颈鳞癌组宫颈脱落细胞中miR-424表达水平依次降低,CUL2 mRNA表达水平依次升高(P均<0.001);HPV16持续感染者宫颈脱落细胞中miR-424和CUL2 mRNA表达水平呈负相关(P<0.001);宫颈脱落细胞中miR-424、CUL2 mRNA、两者联合检测预测宫颈HPV16持续感染的曲线下面积分别为0.871、0.809、0.927。结论 宫颈HPV16持续感染患者宫颈脱落细胞中miR-424表达下降,CUL2 mRNA表达上升,二者呈负相关,且miR-451和CUL2 mRNA联合检测可提高对宫颈HPV16持续感染的预测价值。Objective The predictive value of microRNA-424(miR-424)and Cullin 2(CUL2)messenger RNA(mRNA)expression levels in cervical exfoliated cells for cervical human papilloma virus(HPV)16 persistent infection.Methods From January,2020 to November,2021,259 patients with cervical HPV16 infection who had normal cervical liquid-based cytology in Handan Central Hospital were selected as the research subjects.After 1 year of follow-up,those who remained positive for HPV16 were classified as the HPV16 persistent infection group(136 cases),while those who turned negative for HPV16 were classified as the HPV16 non-persistent infection group(123 cases).Patients with cervical HPV16 persistent infection were grouped by histopathological diagnosis on the day of reexamination:52 patients with chronic cervical inflammation(cervical inflammation group),61 patients with cervical intraepithelial neoplasia(epithelial neoplasia group),and 23 patients with cervical squamous cell carcinoma(cervical squamous cell carcinoma group).Real-time fluorescent quantitative PCR method was used to detect expression levels of miR-424 and CUL2 mRNA in cervical exfoliated cells.Pearson method was used to analyze the correlation between miR-424 and CUL2 mRNA in cervical exfoliated cells of patients with HPV16 persistent infection.Receiver operating characteristic curve was drawn to evaluate the predictive value of miR-424 and CUL2 mRNA expression levels in cervical exfoliated cells for cervical HPV16 persistent infection.Results Compared with the HPV16 non-persistent infection group,the HPV16 persistent infection group had a lower expression level of miR-424 in cervical exfoliated cells,and a higher expression level of CUL2 mRNA(all P<0.001);The expression level of miR-424 in cervical exfoliated cells in cervical inflammation group,epithelial neoplasia group and cervical squamous cell carcinoma group decreased in turn,while the expression level of CUL2 mRNA increased in turn(all P<0.001);Expression levels of miR-424 and CUL2 mRNA in cervical exfoliated cell
