摘 要:目的 分析荧光PCR溶解曲线检测技术在痰液标本中快速鉴定分枝杆菌的应用价值。方法 采用荧光PCR溶解曲线试剂盒进行快速菌种鉴定试验,采用罗氏固体培养管进行传统分枝杆菌分型鉴定。结果 研究发现159份痰标本中MTB复合群有114例,占到了71.7%;MTB+NTM有18例,占到了11.3%;NTM有27例,占到了17.0%。MTB+NTM组与NTM组的菌种存在显著差异。MTB+NTM组中以戈登分枝杆菌为主,而在NTM组中以胞内分枝杆菌及堪萨斯分枝杆菌检出为主。进一步与传统培养鉴定的结果比较发现,荧光PCR溶解曲线检出为MTB的114例标本中,分枝杆菌培养鉴定结果为99例MTB、1例NTM、14例阴性。荧光PCR溶解曲线法检出为MTB+NTM的18例标本中,分枝杆菌培养鉴定结果为14例MTB、1例NTM、3例阴性。荧光PCR溶解曲线检出为NTM的27例标本中,分枝杆菌培养的鉴定结果为5例MTB、19例NTM、3例阴性。结论 荧光PCR溶解曲线检测技术在痰液标本快速鉴定分枝杆菌的应用中具有较好的应用价值,能够为临床快速、精准诊断分枝杆菌病提供帮助。Objective To evaluate the application value of fluorescence PCR dissolution curve detection technique in rapid identification of Mycobacterium sputum samples.Methods Fluorescence PCR dissolution curve kit was used for rapid strain identification test.The traditional mycobacterium typing was identified by Roche solid culture tube.Results There were 114 cases of MTB complex group among 159 sputum specimens,accounting for 71.7%.There were 18 cases of MTB+NTM,accounting for 11.3%.There were 27 cases of NTM,accounting for 17%.There were significant differences in the species composition between the MTB+NTM group and the NTM group.In the MTB+NTM group,Mycobacterium gordonii was mainly detected,while in the NTM group,Mycobacterium intracellular and Mycobacterium kansasensis were mainly detected.Compared with the results of traditional culture identification,the fluorescence PCR dissolution curve identified 114 specimens detected as MTB,in which tuberculosis culture identification results were 99 cases of MTB,1 case of NTM,and 14 cases of negative.Of the 18 samples detected as MTB+NTM by fluorescence PCR dissolution curve method,the identification results of tuberculosis culture were 14 cases of MTB,1 case of NTM,and 3 cases were negative.Among the 27 specimens detected as NTM by fluorescence PCR dissolution curve,the tuberculosis culture was identified as MTB in 5 cases,NTM in 19 cases,and 3 cases were negative.Conclusion Fluorescent PCR dissolution curve detection technology has a good application value in the rapid identification of mycobacteria in sputum samples,and can facilitate the accurate diagnosis of mycobacterial disease in clinical practice.
