摘  要:目的:评价随机对照试验中标准化剂量地塞米松治疗成人急性呼吸窘迫综合征(acute respiratory distress syndrome,ARDS)的疗效。方法:利用计算机检索已公开发表的有关糖皮质激素治疗ARDS的中英文文献,按纳入、排除标准纳入合格文献并进行质量评价,采用RevMan5.0软件进行Meta分析。结果:筛选899篇文献,最终纳入3篇文献进行分析。结果发现标准化剂量地塞米松组与对照组相比,标准化剂量地塞米松组28天内存活患者的非机械通气时间显著增加。但患者的28天全因死亡率、高血糖、新发感染发生率差异无显著性。结论:标准化剂量地塞米松可增加28天内存活患者非机械通气时间,但对28天全因死亡率、高血糖及新发感染发生率无影响。Objective:To evaluate the efficacy of standardized dose dexamethasone in the treatment of acute respiratory distress syndrome(ARDS)in adults in a randomized controlled trial.Methods:The Chinese and English pieces of literature about glucocorticoid therapy for ARDS were retrieved by computer.The qualified works of literature were included according to the inclusion and exclusion criteria and the quality was evaluated.Meta-analysis was performed by Revman 5.0 software.Results:A total of 899 pieces of literature were screened,and 3 pieces of literature were included for analysis.The results showed that the duration of non-mechanical ventilation was significantly increased in the standardized dose dexamethasone group compared with the control group within 28 days.There were no significant differences in 28-day all-cause mortality and mechanical complications between the two groups.Conclusion:Standardized dose dexamethasone increased mechanical ventilation time in patients who survived within 28 days without increasing or decreasing infectious complications or mortality.
