EarWell耳廓矫形器矫治小儿先天性耳廓形态畸形的疗效及其影响因素分析

摘　　要：目的:观察EarWell耳廓矫形器治疗不同类型耳廓形态畸形患儿的疗效。方法:选取先天性耳廓形态畸形患儿80例,共110耳(招风耳15耳、垂耳30耳、杯状耳21耳、隐耳25耳、耳轮畸形19耳),患儿初始矫正年龄均小于6周。比较不同类型耳廓畸形患儿在治疗有效率、矫治时间、并发症发生率上的差异,比较其矫治结束后1个月、3个月的复发率。结果:不同类型耳廓畸形患儿在治疗有效率上比较差异无统计学意义(P>0.05)。耳轮畸形的矫治时间最短,招风耳和杯状耳的矫治时间明显长于其他组,差异有统计学意义(P<0.05)。耳轮畸形和垂耳的并发症发生率较低,招风耳和杯状耳的并发症发生率明显高于其他组,差异有统计学意义(P<0.05)。招风耳、杯状耳患儿矫治结束后1个月、3个月的复发率较高,垂耳和隐耳观察1个月、3个月均无复发。耳廓畸形类型与矫治时间、并发症发生率及复发率之间均有相关性(P<0.05)。结论:EarWell耳廓矫形器对耳廓形态畸形患儿的治疗效果确切,矫治时间、并发症发生率及复发率与畸形类型有关。Objective To observe the efficacy of EarWell auricle orthosis in the treatment of children with difierent types of auricle deformities.Methods 80 children with congenital auricle deformity were selected,a total of 110 ears(15 ears of prominent ear,30 ears of lop ear,21 ears of cup ear,25 ears of cryptotia and 19 ears of helical rim deformity).The initial correction age of the children was less than 6 weeks.To compare the difierences in treatment efficiency,correction time and complication rate of children with different types of auricle malformations,and to compare the recurrence rate at 1 month and 3 months after correction.Results There was no significant difierence in the efiective rate of treatment among children with difierent types of auricle malformations(P>0.05).The correction time of helical rim deformity was the shortest,and the correction time of prominent ear and cup ear was significantly longer than that of other groups(P<0.05).The incidence of complications of ear helical rim deformity and lop ear was lower,and the incidence of complications of prominent ear and cup ear was significantly higher than that of other groups(P<0.05).The recurrence rate of children with prominent ear and cup ear was higher at 1 and 3 months after correction,children with lop ear and cryptotia were no recurrence at 1 and 3 months after treatment.The type of auricle deformity was correlated with the correction time,the incidence of complications and the recurrence rate(P<0.05).Conclusion The treatment efiect of EarWell auricle orthosis on children with auricle morphological malformation is accurate.The correction time,complication rate and recurrence rate are related to the type of malformation.
