摘　　要：目的分享临床药师参与多病共存患者治疗药物管理的经验。方法临床药师通过参与1例多病共存患者药物治疗管理过程,结合患者特点、并存疾病,查阅相关资料和文献后进行用药建议,在保证疗效的同时精简患者的药物种类。结果患者的治疗药物种类减少,节约了治疗药物成本,降低了药物不良反应发生风险。结论临床药师参与临床治疗,对多病共存患者进行治疗药物管理和药学监护,可提高临床药物治疗安全性,降低治疗费用。Objective To share the experience of clinical pharmacists involved in drug management for a patients with multiple diseases.Methods Considering the characteristics of the coexistence of diseases and consulting relevant literature,the clinical pharmacists made a recommendation for a patient with multiple diseases while participating in the therapy process,so as to simplify the drug types of the patient while ensuring the efficacy.Results The variety of treatment drugs for the patient was reduced,which saves the cost of treatment drugs and reduces the risk of adverse drug reactions.Conclusion Participation of clinical pharmacists in clinical treatment,drug management and pharmaceutical monitoring for patients with multiple diseases can improve the safety of clinical drug therapy and reduce the cost of treatment.
