摘 要:目的:探究基底核区脑出血患者采取经脑沟裂入路显微手术治疗的效果。方法:选取2019年3月—2022年6月东莞市人民医院收治的60例基底核区脑出血患者为研究对象,采用随机数表法分为对照组、试验组,各30例。对照组经脑回皮质手术治疗,试验组经脑沟裂入路显微手术治疗。比较两组临床疗效、Barthel指数量表(BI)评分、日常生活活动能力(ADL)评分、并发症发生率及免疫功能指标情况。结果:试验组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。术后4周、12周,试验组BI评分高于对照组,差异有统计学意义(P<0.05)。术后6个月,试验组恢复良好率高于对照组,差异有统计学意义(P<0.05)。试验组并发症发生率为10.00%,低于对照组的26.67%,差异有统计学意义(P<0.05)。术后1 d、7 d,试验组CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)均高于对照组,差异有统计学意义(P<0.05)。结论:经脑沟裂入路显微手术治疗基底核区脑出血的效果显著,能有效改善患者的免疫系统紊乱等情况,减少并发症发生,改善其日常生活活动能力。Objective:To explore the effect of microsurgery via sulcus fissure approach in patients with cerebral hemorrhage in basal ganglia region.Method:A total of 60 patients with cerebral hemorrhage in the basal ganglia region admitted to Dongguan People's Hospital from March 2019 to June 2022 were selected as the objects and they were divided into control group and experimental group using the random number table method,with 30 cases in each group.The control group was treated with transgyri cortical surgery,and the experimental group was treated with microsurgery via sulcus fissure approach.The clinical efficacy,Barthel index scale(BI)score,activities of daily living(ADL)score,complication rate and immune function indicators of the two groups were compared.Result:The total effective rate of treatment in the experimental group was higher than that in the control group,the difference was statistically significant(P<0.05).At 4 weeks and 12 weeks after surgery,the BI score of the experimental group was higher than that of the control group,the difference was statistically significant(P<0.05).At 6 months after surgery,the recovery rate of the experimental group was higher than that of the control group,the difference was statistically significant(P<0.05).The incidence of complications in the experimental group was 10.00%,which was lower than 26.67%in the control group,the difference was statistically significant(P<0.05).At the 1 d and 7 d after surgery,the levels of CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)in the experimental group were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The microsurgery via sulcus fissure approach has a significant effect on cerebral hemorrhage in basal ganglia region,which can effectively improve the patients'immune system disorders,reduce complications,and improve their activities of daily living.
