麦角新碱联合缩宫素预防球囊催产阴道分娩产后出血的临床应用

摘　　要：目的:探讨麦角新碱联合缩宫素预防球囊催产阴道分娩产后出血的效果。方法:回顾性分析2020年1月—2021年10月高州市人民医院收治的60例阴道分娩产妇临床资料,将单用缩宫素预防产后出血的30例设为对照组,将麦角新碱联合缩宫素预防产后出血的30例设为观察组。比较两组产后出血量、住院时间、血流动力学指标、宫底高度、血常规指标、产后出血率及不良反应情况。结果:观察组产后2 h、24 h时出血量分别为(151.49±15.62)mL、(229.48±28.94)mL,少于对照组的(179.84±18.77)mL、(336.17±33.81)mL,住院时间为(3.89±0.42)d,短于对照组的(4.79±0.48)d,差异有统计学意义(P<0.05)。观察组用药1 h后舒张压为(69.69±6.03)mmHg(1 mmHg=0.133 kPa),收缩压为(129.63±9.69)mmHg,高于对照组的(64.85±5.78)mm Hg、(120.96±9.35)mm Hg,心率为(85.16±7.68)次/min,低于对照组的(91.45±7.94)次/min,差异有统计学意义(P<0.05)。产后3 d,观察组宫底高度为(16.15±1.53)cm,低于对照组的(17.82±1.75)cm,血红蛋白(Hb)、红细胞计数(RBC)分别为(113.63±9.85)g/L、(3.88±0.41)×10^(12)/L,高于对照组的(105.31±9.27)g/L、(3.47±0.36)×10^(12)/L,差异有统计学意义(P<0.05)。观察组产后出血率低于对照组,差异有统计学意义(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论:麦角新碱联合缩宫素在球囊催产阴道分娩产后出血预防中效果更佳,能减少产后出血量,稳定血流动力学,促进子宫复旧,安全可靠。Objective:To explore the effect of Ergometrine combined with Oxytocin in preventing postpartum hemorrhage in balloon induced vaginal delivery.Method:Retrospective analysis was made on the clinical data of 60 cases of vaginal delivery in Gaozhou People's Hospital from January 2020 to October 2021.Thirty cases of postpartum hemorrhage prevented by Oxytocin alone were set as the control group,and thirty cases of postpartum hemorrhage prevented by Ergometrine combined with Oxytocin were set as the observation group.The volume of postpartum hemorrhage,hospital stay,hemodynamic indexes,changes in fundal height,blood routine indexes,postpartum hemorrhage rate and adverse reactions were compared between the two groups.Result:The bleeding volume of the observation group at 2 h and 24 h postpartum were(151.49±15.62)mL and(229.48±28.94)mL,which were lower than(179.84±18.77)mL and(336.17±33.81)mL in the control group,and the hospital stay was(3.89±0.42)d,which was shorter than(4.79±0.48)d in the control group,the differences were statistically significant(P<0.05).After 1 h of medication,the diastolic blood pressure and systolic blood pressure in the observation group were(69.69±6.03)mmHg(1 mmHg=0.133 kPa)and(129.63±9.69)mmHg,which were higher than(64.85±5.78)mmHg and(120.96±9.35)mmHg in the control group,and the heart rate was(85.16±7.68)beats/min,which was lower than(91.45±7.94)beats/min in the control group,the differences were statistically significant(P<0.05).Three days after delivery,the fundal height of the observation group was(16.15±1.53)cm,which was lower than(17.82±1.75)cm of the control group,and the hemoglobin(Hb)and red blood cell count(RBC)were(113.63±9.85)g/L,(3.88±0.41)×10^(12)/L,which were higher than(105.31±9.27)g/L,(3.47±0.36)×10^(12)/L in the control group,the differences were statistically significant(P<0.05).The rate of postpartum hemorrhage in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).There was no statisticall
