超声引导内热针靶向松解联合液压分离在冻结肩中的疗效研究

摘　　要：目的对比分析超声引导内热针靶向松解联合液压分离与手法松解治疗冻结肩的临床疗效。方法选取2021年6月—2022年9月于福建医科大学附属闽东医院疼痛科住院的45例冻结肩患者作为研究对象,按随机数表法分为两组,对照组(n=22)给予超声引导内热针靶向松解联合手法松解,研究组(n=23)给予超声引导内热针靶向松解联合关节囊液压分离。比较两组治疗前后VAS评分、肩关节活动度(前屈、外展角度)及CMS评分,并记录不良反应情况。结果治疗前两组VAS评分、肩关节前屈角度及外展角度、CMS评分比较,差异无统计学意义(P>0.05)。治疗后1周、1个月、3个月研究组VAS评分分别为(2.43±1.12)分、(1.56±0.95)分、(0.69±0.64)分,均低于对照组,差异有统计学意义(t=3.325、3.044、2.689,P<0.05);治疗后1周、1个月、3个月研究组CMS评分分别为(77.52±7.99)分、(81.52±5.81)分、(86.61±4.28)分,均高于对照组,差异有统计学意义(t=6.395、4.690、5.248,P<0.05)。治疗后研究组不良反应发生率为0.00%,明显低于对照组的40.91%,差异有统计学意义(χ^(2)=9.344,P<0.05)。结论超声引导内热针靶向松解联合液压分离与手法松解治疗冻结肩疗效对比,发现前者能更快改善冻结肩患者的肩痛、肩关节活动度,恢复肩关节功能,疗效确切且不良反应极少,值得临床推广。Objective To compared and analyzed the clinical efficacy of ultrasound guided targeted release of internal heat needle combined with hydraulic separation and manual release in the treatment of frozen shoulder.Methods A total of forty-five patients with frozen shoulder hospitalized in the Department of Pain of Mindong Hospital Affiliated to Fujian Medical University from June 2021 to September 2022 were selected as the study subjects and divided into two groups according to numerical random method.The control group(n=22)was given ultrasound-guided internal heat acupuncture targeted release combined with manual release,and the study group(n=23)was given ultrasound-guided internal heat acupuncture targeted release combined with hydraulic capsule separation.VAS score,shoulder joint range of motion(flexion,abduction angle)and CMS score were compared between the two groups before and after treatment,and adverse reactions were recorded.Results There was no statistically significant difference in VAS score,shoulder joint flexion angle,abduction angle and CMS score between the two groups before treatment(P>0.05).After 1 week,1 month,and 3 months of treatment,the VAS scores of the study group were(2.43±1.12)points,(1.56±0.95)points,and(0.69±0.64)points,respectively,lower than those of the control group,the difference was statistically significant(t=3.325,3.044,2.689,P<0.05);after 1 week,1 month,and 3 months of treatment,the CMS scores of the study group were(77.52±7.99)points,(81.52±5.81)points,and(86.61±4.28)points,higher than those of the control group,the difference was statistically significant(t=6.395,4.690,5.248,P<0.05).The incidence of adverse reactions in the study group after treatment was 0.00%,significantly lower than 40.91%in the control group,the difference was statistically significant(χ^(2)=9.344,P<0.05).Conclusion Comparing the efficacy of ultrasound-guided internal heat acupuncture targeted release combined with hydraulic separation and manual release in the treatment of frozen shoulder,it was found
