腹腔镜完全腹膜外腹股沟疝修补术对老年腹股沟疝患者术后康复及近期生活质量的影响

摘　　要：目的:研究腹腔镜完全腹膜外腹股沟疝修补术在老年腹股沟疝患者中的应用效果及对术后康复及近期生活质量的影响。方法:选取2020年1月─2022年3月经监利市中医医院普外科医师确认需要接受手术治疗的91例老年腹股沟疝患者为研究对象。以手术方式作为分组依据将患者分为观察组(n=46)和对照组(n=45)。对照组实施常规术式,观察组实施腹腔镜完全腹膜外腹股沟疝修补术。手术结束后,比较两组围手术期指标、生活质量水平及并发症情况。结果:观察组手术时长、疼痛持续时长、住院时长均短于对照组,术中出血量少于对照组,首次排气时间、首次下床时间均早于对照组,差异有统计学意义(P<0.05)。术后1周两组各维度生活质量评分均高于术前,且观察组均高于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为6.52%,低于对照组的22.22%,差异有统计学意义(P<0.05)。结论:在对老年腹股沟疝患者进行手术治疗时,选择腹腔镜完全腹膜外腹股沟疝修补术能够有效提高手术效率及临床疗效,加快康复进展,同时能够避免多数并发症的发生。Objective:To investigate the application effect of laparoscopic total extraperitoneal inguinal hernia repair in elderly patients with inguinal hernia and its influence on postoperative rehabilitation and short-term quality of life.Method:A total of 91 elderly patients with inguinal hernia who were confirmed by General Surgeons in Jianli Traditional Chinese Medicine Hospital from January 2020 to March 2022 were selected as the research objects,they were divided into observation group(n=46)and control group(n=45)according to the surgical method.The control group underwent conventional operation,and the observation group underwent laparoscopic total extraperitoneal inguinal hernia repair.After the operation,perioperative indexes,quality of life and complications were compared between the two groups.Result:The operation duration,pain duration and hospitalization duration in the observation group were shorter than those in the control group,the intraoperative blood loss was less than that in the control group,the first time to exhaust gas and the first time to get out of bed were earlier than those in the control group,the differences were statistically significant(P<0.05).One week after operation,quality of life scores in all dimension of both groups were higher than those before operation,and the observation group were higher than the control group,the differences were statistically significant(P<0.05).The complication rate in the observation group was 6.52%,which was lower than 22.22%in the control group,the difference was statistically significant(P<0.05).Conclusion:In the surgical treatment of elderly patients with inguinal hernia,laparoscopic total extraperitoneal inguinal hernia repair can effectively improve the surgical efficiency and clinical efficacy,accelerate the progress of rehabilitation,and avoid the occurrence of most complications.
