摘　　要：目的:探讨关节镜辅助微创手术治疗复杂性胫骨平台骨折的效果。方法:选取2019年2月—2020年3月宁德市中医院收治的94例复杂性胫骨平台骨折患者为研究对象。根据随机数表法将其分为对照组和观察组,各47例。对照组给予锁定加压钢板内固定治疗,观察组给予关节镜辅助微创手术治疗。比较两组围手术期指标,术前、术后12周膝关节功能,并发症及临床疗效。结果:两组均无切口感染、皮肤坏死、骨折移位、骨折不愈合、下肢深静脉血栓等并发症发生。观察组手术时间、住院时间,骨折愈合时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。术后12周,观察组美国特种外科医院膝关节(hospital for special surgery,HHS)评分明显高于对照组,差异有统计学意义(P<0.05)。观察组优良率明显高于对照组,差异有统计学意义(P<0.05)。结论:关节镜辅助微创手术治疗复杂性胫骨平台骨折患者,不仅可以缩短患者住院时间及手术时间,减少术中出血量,还能促进关节功能恢复。Objective:To explore the effect of arthroscopic assisted minimally invasive surgery in the treatment of complex fracture of the tibial plateau.Method:Ninety-four patients with complex fracture of the tibial plateau admitted to Ningde Traditional Chinese Medicine Hospital from February 2019 to March 2020 were selected as the research objects.According to random number method,the patients were divided into control group and observation group,47 cases in each group.The control group was treated with locking compression steel plate internal fixation,and the observation group was treated with arthroscopic assisted minimally invasive surgery.Perioperative indexes,before and 12 weeks after surgery knee joint function,complications and clinical efficacy were compared between the two groups.Result:There were no complications such as incision infection,skin necrosis,fracture displacement,fracture nonunion and deep venous thrombosis of lower limbs in both groups.The operation time,hospital stay and fracture healing time in the observation group were shorter than those in the control group,and the intraoperative blood loss volume was less than that in the control group,the differences were statistically significant(P<0.05).At 12 weeks after surgery,the America hospital for special surgery(HHS)knee score of the observation group was significantly higher than that of the control group,the difference was statistically significant(P<0.05).The excellent and good rate of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Arthroscopic assisted minimally invasive surgery in the treatment of patients with complex fracture of the tibial plateau can not only shorten the hospital time and operation time,reduce intraoperative blood loss volume,but also promote the recovery of joint function.
