摘　　要：目的:研究微通道经皮肾镜取石术(mPCNL)联合输尿管软镜钬激光碎石术(FURL)治疗复杂性肾结石(CRC)的临床效果。方法:选取2019年12月—2022年12月广安市人民医院收治的80例CRC患者为研究对象。根据随机数表法将其平均划分为单独治疗组和联合治疗组,各40例。单独治疗组行FURL治疗。联合治疗组行mPCNL联合FURL治疗。比较两组围手术期指标,并发症,结石一期清除成功率,术前、术后3 d肾功能、炎症因子。结果:联合治疗组手术时间、住院时间均长于单独治疗组,术中出血量多于单独治疗组,差异有统计学意义(P<0.05)。两组术后并发症率发生率比较,差异无统计学意义(P>0.05)。联合治疗组结石一期清除成功率显著高于单独治疗组,差异有统计学意义(P<0.05)。术后3 d,两组血肌酐、血尿素氮水平均出现略微升高,差异有统计学意义(P<0.05)。两组血肌酐、血尿素氮水平比较,差异无统计学意义(P>0.05)。术后3 d,两组白细胞介素-6(IL-6)和C反应蛋白(CRP)水平比较,差异无统计学意义(P>0.05)。结论:采取mPCNL联合FURL治疗CRC患者,能够有效提高结石一期清除成功率。Objective:To investigate the clinical effect of microchannel percutaneous nephrolithotomy(mPCNL)combined with flexible ureteroscope holmium laser lithotripsy(FURL)in the treatment of complex renal calculi(CRC).Method:A total of 80 CRC patients admitted to Guang’an People's Hospital from December 2019 to December 2022 were selected as the study objects.According to the random number table method,the patients were evenly divided into single treatment group and combined treatment group,40 cases in each group.The single treatment group received FURL treatment.The combined treatment group was treated with mPCNL and FURL.Perioperative indexes,complications,success rate of first-stage stone removal,renal function and inflammatory factors before and 3 d after operation were compared between the two groups.Result:The operation time and hospital stay of the combined treatment group were longer than those of the single treatment group,and the intraoperative blood loss volume was more than that of the single treatment group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).The success rate of first-stage stone removal in the combined treatment group was significantly higher than that in the single treatment group,and the difference was statistically significant(P<0.05).At 3 d after operation,the levels of serum creatinine and blood urea nitrogen were slightly increased in the two groups,the differences were statistically significant(P<0.05),but there was no statistical significance in the levels of serum creatinine and blood urea nitrogen between the two groups(P>0.05).At 3 d after operation,there were no significant differences in interleukin-6(IL-6)and C-reactive protein(CRP)levels between the two groups(P>0.05).Conclusion:mPCNL combined with FURL in the treatment of CRC patients can effectively improve the success rate of first-stage stone removal.
