慢性乙型肝炎患者长期使用富马酸替诺福韦二吡呋酯片的不良反应及相关因素分析

摘　　要：目的:探讨慢性乙型肝炎患者长期使用富马酸替诺福韦二吡呋酯片的不良反应,并进行相关因素分析。方法:回顾性分析2021年1月—12月我院诊治的96例慢性乙型肝炎患者临床资料,所有患者均给予富马酸替诺福韦二吡呋酯片治疗,根据患者治疗期间出现的不良反应情况分为不良反应组和非不良反应组。采用单因素、多因素及相关性分析患者使用富马酸替诺福韦二吡呋酯片不良反应的因素。结果:96例在治疗3个月期间发生不良反应9例,占比9.4%,其中瘙痒3例、胃肠道反应3例、头痛2例、失眠1例。两组年龄、病程、Child-Pugh评分、焦虑SAS评分、抑郁SDS评分比较,差异有统计学意义(P<0.05);Logistic回归模型显示,年龄、病程、Child-Pugh评分、焦虑SAS评分、抑郁SDS评分为导致慢性乙型肝炎患者长期使用富马酸替诺福韦二吡呋酯片发生不良反应的重要因素(P<0.05);Spearman分析显示,不良反应与年龄、病程、Child-Pugh评分、焦虑SAS评分、抑郁SDS评分呈正相关(P<0.05)。结论:慢性乙型肝炎患者长期使用富马酸替诺福韦二吡呋酯片具有很好的治疗效果,但不良反应发生率依然较高,年龄、病程、Child-Pugh评分、焦虑SAS评分、抑郁SDS评分等都为导致患者出现不良反应的重要因素。Objective:To investigate the adverse reactions of long-term use of tenofovir disoproxil fumarate tablets in patients with chronic hepatitis B,and to analyze its related factors.Methods:The clinical data of 96 patients with chronic hepatitis B diagnosed and treated in our hospital from January 2021 to December 2021 were retrospectively analyzed.All patients were treated with tenofovir disoproxil fumarate tablets.According to the adverse reactions during treatment,the patients were divided into adverse reaction group and non-adverse reaction group.Univariate,multivariate and correlation analysis were used to analyze the factors of adverse reactions of tenofovir disoproxil fumarate tablets.Results:Nine cases(9.4%)of 96 cases experienced adverse reactions during 3 months of treatment,including 3 cases of pruritus,3 cases of gastrointestinal reactions,2 cases of headache,and 1 case of insomnia.There were significant differences in age,course of disease,Child-Pugh score,anxiety SAS score and depression SDS score between the two groups(P<0.05).Logistic regression model showed that age,course of disease,Child-Pugh score,anxiety SAS score and depression SDS score were important factors leading to adverse reactions of long-term use of tenofovir disoproxil fumarate tablets in patients with chronic hepatitis B(P<0.05).Spearman analysis showed that adverse reactions were positively correlated with age,course of disease,Child-Pugh score,anxiety SAS score and depression SDS score(P<0.05).Conclusion:Long-term use of tenofovir disoproxil fumarate tablets in patients with chronic hepatitis B has a good therapeutic effect,but the incidence of adverse reactions is still high.Age,course of disease,Child-Pugh score,anxiety SAS score and depression SDS score are all important factors leading to adverse reactions in patients.
