19例急性化脓性扁桃体炎咽拭子细菌培养及药敏分析

摘　　要：目的:分析成人急性化脓性扁桃体炎的致病菌种类及其耐药性。方法:对2020年8月—2021年11月至我科门诊就诊的19例急性化脓性扁桃体炎的患者用咽拭子采集扁桃体表面的脓性分泌物进行细菌培养及药敏试验并对结果进行分析。结果:19例患者中草绿色链球菌占比63.2%,奈瑟菌占比57.9%,化脓性链球菌占比26.3%,咽峡炎链球菌、肠球菌占比各5%。化脓性链球菌与咽峡炎链球菌对克林霉素及红霉素耐药率极高,但对青霉素及头孢菌素敏感。有1例患者同时患有传染性单核细胞增多症。结论:对于急性化脓性扁桃体炎的患者,草绿色链球菌和奈瑟菌等机会致病菌的药敏测试是有意义的。急性化脓性扁桃体炎与EB病毒之间的关系有待进一步研究。Objective:To analyze the pathogen types and pathogens’drug resistance of adult acute suppurative tonsillitis.Methods:In our outpatient department,19 cases of acute suppurative tonsillitis were collected by throat swabs for bacterial culture and microbial sensitivity tests from Aug 2020 to Nov 2021,and the results were analyzed.Results:The bacterial culture and microbial sensitivity tests were as follows:Streptococcus viridans accounted for 63.2%,Neisseria accounted for 57.9%,Pyogenic streptococci accounted for 26.3%,Streptococcus anginosus accounted for 5%,Enterococcus accounted for 5%.Pyogenic streptococci and Streptococcus anginosus were highly resistant to erythromycin and clindamycin,but they were sensitive to cephalosporins and penicillin.One patient had both acute suppurative tonsillitis and infectious mononucleosis.Conclusion:Microbial sensitivity test for opportunistic pathogens such as Streptococcus viridans and Neisseria is significant for patients with acute suppurative tonsillitis.The relationship between acute suppurative tonsillitis and epstein-barr virus needs further study.
