维A酸类制剂对皮肤的刺激作用及其影响因素分析

摘　　要：目的:探讨维A酸类制剂对皮肤的刺激作用及其影响因素。方法:选取2019年1月—2021年12月本院招募的37例受试者,采用随机方法分为试验药组(30例痤疮受试者)和对照组(7例健康受试者)。试验药组给予不同的试验药(a~f组),对照组采用白凡士林,对受试者进行每周每种药品3次24 h的斑贴试验和1次72 h的斑贴试验,观察各组试验效果,并分析维A酸类制剂对皮肤刺激作用的影响因素。结果:b组平均累计刺激指数高于其他各组(P<0.05);e组平均累计刺激指数高于a组、f组、d组和对照组(P<0.05);d组平均累计刺激指数高于对照组(P<0.05);对照组平均累计刺激指数低于f组、a组,但与c组相近(P>0.05)。b组在第15天时出现渗液1例,第12天时出现水疱1例;d组在第4、5、8、10天时均出现渗液1例;f组在第2、5、9天时均出现水疱1例。维A酸类制剂的剂量、种类、浓度、患者使用时间为影响维A酸类制剂对皮肤的刺激作用的主要因素。结论:0.1%维A酸霜剂对于皮肤的刺激效果较小,同时对皮肤的刺激作用受维A酸类制剂的剂量、种类、浓度、患者使用时间影响,要合理用药,减少对皮肤的刺激作用。Objective:To explore the irritating effect of retinoic acid preparations on the skin and its influencing factors.Methods:A total of 37 subjects recruited by the hospital from January 2019 to December 2021 were randomly divided into an experimental drug group(30 acne subjects)and a control group(7 healthy subjects).The experimental drug group was given different experimental drugs(a-f groups),while the control group was given white Vaseline.Three 24-hour patch tests and one 72-hour patch test were conducted on each drug per week to observe the effectiveness of each group,and analyzed the influencing factors of retinoic acid preparations on skin irritation.Results:The average cumulative stimulus index of group b was higher than that of other groups(P<0.05).The average cumulative stimulus index of group e was higher than that of groups a,f,d,and control group(P<0.05).The average cumulative stimulus index of group d was higher than that of the control group(P<0.05).The average cumulative stimulus index of the control group was lower than that of groups f and a,but similar to group c(P>0.05).Group b had 1 case of fluid leakage on the 15th day and 1 case of blister on the 12th day.On the 4th,5th,8th,and 10th day,there was 1 case of fluid leakage in group d.One case of blister appeared in group f on days 2,5,and 9.The dosage,type,concentration,and patient use time of retinoic acid preparations were the main factors that affect the skin irritation of retinoic acid preparations.Conclusion:0.1%retinoic acid cream has a less irritating effect on the skin,and its irritating effect on the skin is influenced by the dosage,type,concentration,and patient use time of retinoic acid preparations.It is necessary to use them reasonably to reduce their irritating effect on the skin.
