41例儿童外阴硬化性苔藓皮肤镜特征分析

摘　　要：目的分析儿童外阴硬化性苔藓患者的皮肤镜特征。方法回顾性分析41例于苏州大学附属儿童医院皮肤科诊治的外阴硬化性苔藓儿童患者的皮肤镜影像资料。结果皮肤镜结果显示,41例(100%)皮损中均见到黄白色无结构区域及线状血管,30例(73.17%)见毛囊角栓,26例(63.41%)见蓝灰色色素呈胡椒粉样分布,15例(36.59%)见斑片状色素沉着,11例(26.83%)见紫红色小球或斑片。结论在皮肤镜下见到黄白色结构、线状血管、毛囊角栓、蓝灰色色素斑点等有助于儿童外阴硬化性苔藓的诊断。Objective To analyze dermoscopic features of vulvar lichen sclerosus in children.Methods The dermoscopic images of vular lichen sclerosus were retrospectively analyzed in 41 children who were treated at our department.Results All cases exhibited yellowish-white amorphous areas and linear vessels.Follicular keratotic plugs were seen in 30 cases(73.17%),while patients with blue-gray pepper-like granules accounted for 63.41%(26 cases).Other features included pigmented macules in 15 cases(36.59%)and reddish-purplish globules or patches in 11 cases(26.83%).Conclusions Under dermoscopy,the features of yellowish-white amorphous areas,linear vessels,follicular keratotic plugs,and bluish-gray granules are helpful for the diagnosis of childhood vulvar lichen sclerosus.
