平阳霉素局部注射致过敏性休克1例

摘　　要：报告1例使用平阳霉素局部注射治疗跖疣致过敏性休克。患者男,30岁,因足跖前部出现两个直径分别约1 cm及1 mm的角化性丘疹2个月就诊,根据皮疹特征诊断为跖疣。患者既往体健,否认食物、药物过敏史。2021年5月于我院接受平阳霉素局部注射治疗,注射间隔时间为3~4周,3次治疗后因效果显著,肉眼不可见皮损,未予继续治疗。2022年7月因复发,再次就诊我院接受第四次平阳霉素注射治疗,但8月行第五次注射完毕后约5 min出现视物模糊、呼吸困难、血压下降等过敏性休克症状,立即予静推地塞米松5 mg及肌肉注射盐酸肾上腺素0.5 mg等抢救措施,约1 h后症状逐渐缓解,生命体征平稳出院。To report a case of anaphylactic shock caused by the treatment of plantar warts with local injection of pingyangmycin and review the relevant literature.A 30-year-old male had two horny growths on the front sole for 2 months and was diagnosed with plantar wart.Patient was healthy without a history of allergy to foods or drugs.He was treated with local injection of Pingyangmycin in the interval of 3~4 weeks at our hospital in May 2021.After three treatments,the patient did not continue the treatment because of the disappearance of the lesions.In July 2022,plantar wart recurred and the patient received the injection of pingyangmycin again.However,about 5 minutes after the fifth injection,patient developed symptoms of anaphylactic shock such as blurred vision,dyspnea,and blood pressure dropping.Dexamethasone(5 mg IV)and epinephrine(0.5 mg IM)were given immediately.About 1 hour later,the symptoms were gradually relieved and his vital signs were stable.
