清心脾颗粒联合常规西药治疗心脾实热证小儿疱疹性口腔炎临床研究

摘　　要：目的:观察清心脾颗粒联合常规西药治疗心脾实热证小儿疱疹性口腔炎的临床疗效。方法:选取80例心脾实热证疱疹性口腔炎患儿,按数字奇偶法随机分为治疗组与对照组各40例。对照组给予单磷酸阿糖腺苷、溶菌酶肠溶片等常规西药治疗,治疗组在对照组基础上给予清心脾颗粒治疗。治疗7 d后评估2组临床疗效、中医证候积分、症状缓解时间及血清炎症因子[C-反应蛋白(CRP)、白细胞介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)]水平。结果:治疗组总有效率为97.50%,对照组为85.00%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组中医证候积分均较治疗前降低(P<0.05),且治疗组中医证候积分低于对照组(P<0.05)。治疗后,治疗组退热时间、疱疹消退时间、疼痛缓解时间、饮食恢复时间均短于对照组,差异均有统计学意义(P<0.05)。治疗后,2组CRP、TNF-α水平均较治疗前降低(P<0.05),IL-10水平较治疗前升高(P<0.05);且治疗组CRP、TNF-α水平低于对照组(P<0.05),IL-10水平高于对照组(P<0.05)。结论:清心脾颗粒联合常规西药治疗心脾实热证小儿疱疹性口腔炎疗效确切,可减轻机体炎症反应,促进临床症状体征缓解。Objective:To observe the clinical effect of Qingxinpi Granules combined with routine western medicine for herpetic stomatitis in children with syndrome of excess heat in the heart and spleen.Methods:A total of 80 cases of children with herpetic stomatitis and syndrome of excess heat in the heart and spleen were selected and divided into the treatment group and the control group according to the random odd-even number method,with 40 cases in each group.The control group was treated with Arabinoid Adenosine Monophosphate,Lysozyme Enteric Coated Tablets,and other routine western medicines,and the treatment group was additionally treated with Qingxinpi Granules based on the treatment of the control group.After 7-day treatment,clinical effects,traditional Chinese medicine(TCM)syndrome scores,the remission time of symptoms,and the levels of serum inflammatory factors including C-reactive protein(CRP),interleukin-10(IL-10),and tumor necrosis factor-α(TNF-α)were evaluated in the two groups.Results:The total effective rate was 97.50%in the treatment group and85.00%in the control group,the difference being significant(P<0.05).Before treatment,there was no significant difference being found in the comparison of TCM syndrome scores between the two groups(P<0.05).After treatment,TCM syndrome scores in the two groups were decreased when compared with those before treatment(P<0.05),and the TCM syndrome scores in the treatment group were lower than those in the control group(P<0.05).After treatment,the time to reduce the fever,resolve herpes,relieve pain,and resumed diet in the treatment group was shorter than that in the control group,differences being significant(P<0.05).After treatment,the levels of CRP and TNF-αin the two groups were decreased when compared with those before treatment(P<0.05),and the levels of IL-10 were increased when compared with those before treatment(P<0.05);the levels of CRP and TNF-αin the treatment group were lower than those in the control group(P<0.05),and the IL-10 level in the treatment group
