调理脾肾法针刺联合常规西药治疗脾肾亏虚型特发性膜性肾病临床研究

摘　　要：目的:观察调理脾肾法针刺联合常规西药治疗脾肾亏虚型特发性膜性肾病的疗效。方法:选取118例脾肾亏虚型特发性膜性肾病患者开展前瞻性研究,按随机数字表法分为2组各59例。对照组给予常规西药治疗,联合组给予调理脾肾法针刺联合常规西药治疗。2组均治疗6个月。治疗前后评定中医证候评分,检测24 h尿蛋白定量(24h-UTP)、肾功能指标[血清肌酐(SCr)、血清白蛋白(Alb)]、血脂指标[血清甘油三酯(TG)、总胆固醇(TC)],比较2组临床疗效及不良反应发生率。结果:治疗后,联合组临床疗效总有效率高于对照组(P<0.05)。治疗3个月,2组中医证候积分、24h-UTP、SCr、TG、TC水平均较治疗前降低(P<0.05),Alb水平均较治疗前升高(P<0.05);联合组中医证候积分、24h-UTP、SCr、TG、TC水平均低于对照组(P<0.05),Alb水平高于对照组(P<0.05)。治疗6个月,2组中医证候积分、24h-UTP、SCr、TG、TC水平均较本组治疗前与治疗3个月降低(P<0.05),联合组中医证候积分、24h-UTP、SCr、TG、TC水平均低于对照组(P<0.05),Alb水平高于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:调理脾肾法针刺联合常规西药治疗脾肾亏虚型特发性膜性肾病可以有效提高肾功能、减少蛋白尿、调节血脂、缓解症状,安全性较好。Objective:To observe the curative effect of acupuncture method of regulating spleen and kidney combined with routine western medicine for idiopathic membranous nephropathy of spleen-kidney deficiency type.Methods:A total of 118 cases of patients with idiopathic membranous nephropathy of spleen-kidney deficiency type were selected for a prospective study,and they were divided into two groups according to the random number table method,with 59 cases in each group.The control group was treated with routine western medicine,and the combination group was treated with acupuncture method of regulating spleen and kidney combined with routine western medicine.Both groups were treated for6 months.Before and after treatment,the traditional Chinese medicine(TCM)syndrome scores were evaluated,and 24-hour urine protein(24h-UTP),kidney function indexes[serum creatinine(SCr),serum albumin(Alb)]and blood lipid indexes[triglyceride(TG),total cholesterol(TC)]were detected.The clinical effects and incidence of adverse reactions were compared between the two groups.Results:After treatment,the total effective rate in the combination group was higher than that in the control group(P<0.05).After a 3-month treatment,the TCM syndrome scores and the levels of 24h-UTP,SCr,TG and TC in the two groups were decreased when compared with those before treatment(P<0.05),and the Alb levels were increased when compared with those before treatment(P<0.05).The TCM syndrome scores and the levels of 24h-UTP,SCr,TG and TC in the combination group were lower than those in the control group(P<0.05),and the Alb level was higher than that in the control group(P<0.05).After a 6-month treatment,the TCM scores and the levels of 24h-UTP,SCr,TG and TC in the two groups were decreased when compared with those after a 3-month treatment(P<0.05).The TCM syndrome scores and the levels of 24h-UTP,SCr,TG and TC in the combination group were lower than those in the control group(P<0.05),and the Alb level was higher than that in the control group(P<0.05).There was no stat
