彩色多普勒超声对卵巢子宫内膜异位囊肿诊断及临床价值

摘　　要：目的探讨彩色多普勒超声检测诊断卵巢子宫内膜异位囊肿(OEC)的临床价值。方法选取2018年4月至2021年4月本院诊治的64例OEC患者为研究对象。采取彩色多普勒超声测定手术后的子宫动脉血流变化[搏动指数(PI)、阻力指数(RI)、动脉收缩期峰值流速/舒张末期血流速度值(S/D)],并分析术后妊娠情况。结果术前,囊肿侧子宫动脉的PI(0.91±0.23)、RI(2.58±0.49)、S/D(7.35±0.95)高于无囊肿侧子宫动脉的PI(0.83±0.15)、RI(1.89±0.36)、S/D(5.23±0.67),差异有统计学意义(P<0.05);术后,囊肿侧子宫动脉与无囊肿侧子宫动脉的PI、RI、S/D相比,差异无统计学意义(P>0.05);以RI<0.8为正常依据,术前无囊肿侧RI<0.8者占比89.06%(57/64),高于囊肿侧的25.00%,差异有统计学意义(P<0.05);术后,无囊肿侧与囊肿侧RI<0.8占比相比,差异无统计学意义(P>0.05);40例不孕者中术后RI<0.8的31例,RI≥0.8的9例,RI<0.8的不孕者妊娠率为87.10%,高于RI≥0.8的33.33%(3/9),差异有统计学意义(P<0.05)。结论彩色多普勒超声检测在OEC诊断中临床价值较高,根据子宫动脉血流变化还能预测术后妊娠情况,可进行大力推广。Objective To investigate the clinical value of color Doppler ultrasound in diagnosing ovarian endometriotic cyst(OEC).Methods 64 patients with OEC diagnosed and treated in the hospital from April 2018 to April 2021 were selected as the research objects.The changes of uterine artery blood flow(pulsation index(PI),resistance index(RI),peak systolic flow rate/end diastolic flow rate(S/D)after operation were measured by color Doppler ultrasound,and the pregnancy was analyzed.Results Before operation,PI(0.91±0.23),RI(2.58±0.49),S/D(7.35±0.95)of the cystic uterine artery were significantly higher than those of the non cystic uterine artery(0.83±0.15),(1.89±0.36),(5.23±0.67)(P<0.05);There was no significant difference in PI,RI,S/D between the cystic uterine artery and the non cystic uterine artery after operation(P>0.05);Based on RI<0.8 as the normal basis,the proportion of patients with RI<0.8 on the non cyst side before operation 89.06%was higher than that on the cyst side 25.00%,with a statistical difference(P<0.05);After operation,there was no significant difference in RI<0.8 between the non cyst side and the cyst side(P>0.05);In 40 cases of infertility,31 cases had RI<0.8,9 cases had RI≥0.8.The pregnancy rate of infertility with RI<0.8 was 87.10%,which was higher than 33.33%with RI≥0.8(P<0.05).Conclusion Color Doppler ultrasound of uterine artery blood flow is of high clinical value in the diagnosis of OEC,and it can also predict the pregnancy situation according to the changes of uterine artery blood flow,which can be vigorously promoted.
