熊国良辨治IgA肾病用药规律分析

摘　　要：目的:运用数据挖掘探讨深圳名中医熊国良教授治疗IgA肾病的治法治则及用药特色,为中药治疗IgA肾病提供参考。方法:收集2017年1月-2021年9月于熊国良教授门诊治疗的IgA肾病病例处方,录入中医传承辅助平台,对药物频数、性味及归经等进行描述性分析,并进行关联规则及系统聚类分析。结果:符合标准IgA肾病患者共计116例,中药数共计90味,排名前16位依次为黄芪、丹参、山茱萸、熟地黄、紫苏叶、莲须、金樱子、芡实、蝉蜕、薏苡仁、赤小豆、益母草、白茅根、小蓟炭、鬼箭羽、蝉花。用药性味以温、平性及甘、苦、涩味为主,主要归肝、肺、脾、肾经,其次在心经。关联规则得出常用的核心药物组合8对,演化得到潜在组合8个;聚类得到新处方4首。结论:熊国良教授治疗IgA肾病以“五脏相关学说”为核心思想,强调标本同治,整体观念,注重补肝、宣肺、健脾、益肾、养心,随症搭配活血化瘀、祛风解表、清热祛湿之品,喜用甘温药物。Objective:Data mining was applied to explore the treatment principle and the medication characteristics of IgA Nephropathy by Professor Xiong Guoliang,a famous doctor of traditional Chinese medicine in Shenzhen,to provide a basis and reference ideas for the in-depth study of traditional Chinese medicine in the treatment of IgA nephropathy.Methods:The prescriptions who were diagnosed with IgA nephropathy from January 2017 to September 2021 in Professor Xiong Guoliang's outpatient service were collected.Then the traditional Chinese medicines were entered into the Traditional Chinese Medicine Inheritance Support System.Descriptive analysis was carried out on drug frequency,nature,taste,and meridian,and association rules and systematic cluster analysis were carried out.Results:A total of 116 patients with IgA nephropathy were enrolled and the total number of applied traditional Chinese medicines was 90.The top 16 medicines were Astragali Radix,Salviae Miltiorrhizae Radix et Rhizoma,Corni Fructus,RehmannIiae Radix Praeparata,Perillae Folium,Nelumbints Stamen,Rosae Laevigatae Fructus,Euryales Semen,Cicadae Periostracum,Coicis Semen,Vignae Semen,Leonuri Herba,Imperatae Rhizoma,Cirsii Japonici Herba Carbonisata,Euonymus alatus,Isaria cicadae Miquel.The medicines were mainly warm and neutral in nature,and their tastes were sweet,bitter,and astringent.They mainly belonged to liver,lung,spleen,and kidney meridians,followed by heart meridians.8 pairs of common core drug pairs were obtained by association rules,and 8 potential combinations were evolved;Four new prescriptions were obtained by cluster analysis.Conclusion:Professor Xiong values the theory of“Correlation of Five Viscera”and emphasizes the holistic concept,both manifestation and the root cause of the disease in treating IgA nephropathy.Professor Xiong pays attention to tonifying the liver,diffusing the lung,strengthening the spleen,tonifying the kidney,and nourishing the heart.He matches the products of promoting blood circulation,removing blood stasis,dispell
