摘 要:目的探讨念珠菌阴道炎(VVC)患者和健康者阴道分泌物中微生物结构、组成、功能及代谢的差异。方法选取2020年12月至2022年12月四川省自贡市第四人民医院收治的VVC患者100例纳入研究组,另选取100例健康女性纳入对照组。收集所有受试者阴道分泌物,通过宏基因组测序分析微生物类型和微生物基因组数据,明确差异性和功能性。结果比较两组女性的基线资料,体质量指数(BMI)、Amsel标准(乳灰色分泌物、平均阴道pH、阳性嗅觉试验和线索细胞阳性)和Nugent评分差异具有统计学意义(P<0.05)。研究组阴道微生物组的多样性显著低于对照组,微生物基因组丰度显著低于对照组(P<0.05)。通过微生物差异性分析发现,选取16种差异性微生物,其中4种菌株在对照组阴道中未发现,而在研究组阴道中明显存在且丰度较高;8种菌株在对照组中存在且丰度较高,但在研究组阴道中没有检测到;其余4种菌株丰度差异较大。阴道微生物的功能及代谢存在明显的差异性。结论女性阴道微生物群中的细菌在健康和体内平衡中起着重要作用,更好地了解正常和健康的阴道生态系统,基于其真实功能,将有助于更好地定义健康,进一步改善疾病诊断,以及制定更个性化的方案以促进健康和治疗疾病。Objective To investigate the differences in the structure,composition,function and metabolism of microorganisms in vaginal secretions of patients with vulvovaginitis candidasis(VVC)and healthy individuals.Methods One hundred patients with VVC admitted to Zigong Fourth People’s Hospital of Sichuan Province from December 2020 to December 2022 were selected to be included in the study group,and another 100 healthy women were selected to be included in the control group.Collection of vaginal secretions from all subjects and analysis of microbial types and microbial genomic data by macrogenomic sequencing to clarify variability and functionality.Results Comparing baseline information between the two groups of women,the differences in body mass index(BMI),Amsel criteria(breast grey discharge,mean vaginal pH,positive sniff test and positive clue cells)and Nugent score were statistically significant(P<0.05).The diversity of the vaginal microbiome was significantly lower in the study group than in the control group,and the abundance of the microbial genome was significantly lower than in the control group(P<0.05).The microbial differential analysis revealed that 16 differential microorganisms were selected,of which 4 strains were not found in the control vagina but were significantly present and in higher abundance in the study group vagina.8 strains were present and in higher abundance in the control group but not detected in the study group vagina;the remaining 4 strains differed significantly in abundance.There was significant variability in the function and metabolism of vaginal microorganisms.Conclusion Bacteria in the female vaginal microbiota play an important role in health and homeostasis in vivo,and a better understanding of the normal and healthy vaginal ecosystem,based on its true function,will help to better define health,further improve disease diagnosis,and develop more personalized protocols to promote health and treat disease.
