老年股骨颈骨折患者全身麻醉术后认知功能障碍发生的影响因素

摘　　要：目的:探究老年股骨颈骨折患者全身麻醉术后发生认知功能障碍(POCD)的影响因素.方法:选择2019年1月至2020年12月本院收治的43例全身麻醉术后发生POCD的老年股骨颈骨折患者为研究对象,将其纳入研究组;另选择同期本院收治的43例全身麻醉术后未发生POCD的老年股骨颈骨折患者,纳入对照组.查阅患者临床资料,将可能影响老年股骨颈骨折全身麻醉术后发生POCD的影响因素进行多因素非条件Logistic回归分析.结果:患者合并原发性高血压、手术时间、术后疼痛比较,差异均有统计学意义(P<0.05);经多因素非条件Logistic回归分析结果显示,合并原发性高血压、手术时间长、术后重度疼痛均是导致老年股骨颈骨折患者全身麻醉术后发生POCD的危险因素(OR>1,P<0.05).结论:老年股骨颈骨折患者全身麻醉术后发生POCD受合并原发性高血压、手术时间长、术后重度疼痛等因素影响.Objective:To explore influencing factors of postoperative cognitive dysfunction(POCD)in elderly patients with femoral neck fracture after general anesthesia.Methods:A total of 43 elderly femoral neck fracture patients with POCD after general anesthesia in our hospital from January 2019 to December 2020 were selected and included in occurrence group.In addition,43 elderly femoral neck fracture patients without POCD after general anesthesia in same period were selected and included in non-occurrence group.The clinical data of patients were reviewed,and possible influencing factors that may lead to POCD in elderly patients with femoral neck fracture after general anesthesia were analyzed.Results:There were statistical significant differences in hypertension,operation time and postoperative pain in both groups(P<0.05).Multivariate unconditional Logistic regression analysis showed that hypertension,long operation time and severe postoperative pain were risk factors for POCD in elderly patients with femoral neck fracture after general anesthesia(OR>1,P<0.05).Conclusion:POCD in elderly patients with femoral neck fracture after general anesthesia is affected by hypertension,long operation time,severe postoperative pain and other factors.
