心电图QT间期改变对预测变异性心绞痛患者发生室性心律失常的价值研究

摘　　要：目的:分析心电图QT间期改变预测变异性心绞痛患者发生室性心律失常的应用价值.方法:前瞻性纳入信阳市中心医院20120年8月至2022年8月收治的68例变异性心绞痛患者,所有患者入院次日均行24h动态心电检测.连续随访3个月,记录室性心律失常发生情况.将发生室性心律失常的16例患者纳入发生组,将未发生室性心律失常的52例患者纳入未发生组.分析心电图QT间期改变预测变异性心绞痛患者发生室性心律失常的价值.结果:发生组QT值明显低于未发生组,差异有统计学意义(P<0.05);组间其他资料相比,差异无统计学意义(P>0.05).绘制受试者工作特征(ROC)曲线显示,心电图QT值预测变异性心绞痛患者发生室性心律失常的曲线下面积(AUC)为0.854(95%CI:0.704~0.933).结论:心电图QT间期改变对变异性心绞痛患者发生室性心律失常有良好的预测价值.Objective:To analyze the application value of QT interval change of electrocardiogram in the prediction of ventricular arrhythmia in patients with variant angina pectoris.Methods:A total of 68 patients with variant angina pectoris admitted to Xinyang Central Hospital from August 2020 to August 2022 were prospectively included.All patients were examined by ambulatory electrocardiogram on the next day after admission.During a continuous follow-up period of 3 months,the occurrence of ventricular arrhythmias was recorded.16 patients with ventricular arrhythmias were included in the occurrence group,and 52 patients without ventricular arrhythmias were included in the non-occurrence group.The value of QT interval changes of electrocardiogram in predicting ventricular arrhythmia in patients with variant angina pectoris was analyzed.Results:The QT value of the occurrence group was lower than that of the non-occurrence group,the difference was statistically significant(P<0.05);there was no statistical significant difference in other data between the two groups(P>0.05).The receiver operating characteristic(ROC)curve was drawn and showed that the area under the curve(AUC)of QT value of electrocardiogram in predicting ventricular arrhythmia in patients with variant angina pectoris was 0.854(95%CI:0.704~0.933).Conclusion:QT interval change of electrocardiogram has good predictive value for ventricular arrhythmia in patients with variant angina pectoris.
