不同剂量胺碘酮联合美托洛尔治疗心房纤颤患者的临床疗效及安全性分析

摘　　要：目的:分析不同剂量胺碘酮联合美托洛尔治疗心房纤颤患者的临床疗效及安全性.方法:选取2019年2月至2021年3月本院收治的心房纤颤患者82例作为研究对象.根据药物使用剂量将其分为小剂量组39例(胺碘酮200mg,bid)和大剂量组43例(胺碘酮200mg,tid),均联合美托洛尔治疗.对比两组临床疗效、复律率及复律时间、不良反应发生率.结果:大剂量组临床总有效率(90.70%,39/43)高于小剂量组(71.80%,28/39)(P<0.05);大剂量组完全复律34例(占79.07%),小剂量组完全复律27例(占69.23%),两组复律率比较,差异无统计学意义(P>0.05).大剂量组完全复律时间短于小剂量组(P<0.05);大剂量组不良反应发生率(9.30%,4/43)与小剂量组(5.13%,2/39)比较,差异无统计学意义(P>0.05).结论:大剂量胺碘酮联合美托洛尔治疗心房纤颤患者,能有效提高临床疗效,在一定时间恢复维持窦性心率,且安全性高.Objective:To analyze the clinical efficacy and safety of different doses of amiodarone combined with metoprolol in the treatment of patients with atrial fibrillation.Methods:Eighty-two patients with atrial fibrillation admitted to our hospital from February 2019 to March 2021 were selected in this study.And according to drug doses,they were divided into the low-dose group with 39 cases(amiodarone 200mg,bid)and the high-dose group with 43 cases(amiodarone 20mg,tid),and both groups were treated with metoprolol.The clinical efficacy,cardioversion rate,cardioversion time,and incidence of adverse reactions were compared between the two groups.Results:The total clinical effective rate in the high-dose group(90.70%,39/43)was higher than that in the low-dose group(71.80%,28/39)(P<0.05);there were 34 cases(79.07%)of complete cardioversion in the high-dose group,and 27 cases(69.23%)in the low-dose group,and there was no significant difference in the rate of cardioversion between the two groups(P>0.05).The complete cardioversion time in the high-dose group was shorter than that in the low-dose group(P<0.05);and there was no significant difference in the incidence of adverse reactions between the high-dose group(9.30%,4/43)and the low-dose group(5.13%,2/39)(P>0.05).Conclusion:High-dose amiodarone combined with metoprolol in the treatment of patients with atrial fibrillation can effectively improve the clinical efficacy,restore and maintain sinus heart rate within a certain period of time,and has high security.
