摘 要:目的 探讨血清纤维蛋白原(FIB)、血管生成素2(Ang2)及尿激酶型纤溶酶原激活剂受体(uPAR)表达水平与系统性红斑狼疮(SLE)患者疾病活动度的相关性。方法 选取2020年2月—2022年6月医院收治的SLE患者65例,依据SLE疾病活动指数(SLEDAI)积分将SLE患者分为中重度组(SLEDAI≥10分)(n=34)和轻度组(SLEDAI<10分)(n=31),统计2组临床资料,分析血清FIB、Ang2及uPAR表达水平与SLE患者疾病活动度的相关性。结果 65例SLE患者中,中重度SLE疾病活动度发生率为52.31%,轻度SLE疾病活动度发生率为47.69%;单因素分析显示,2组肾损害、血清FIB、Ang2及uPAR水平比较,差异均有统计学意义(P<0.05);logistic回归分析显示,血清FIB、uPAR表达水平升高,Ang2表达水平降低是SLE患者疾病活动度加重的独立危险因素(P<0.05);Spearman分析显示,血清FIB、uPAR表达水平与SLEDAI成正相关关系(r=0.382、r=0.748,P<0.05),血清Ang2表达水平与SLEDAI成负相关关系(r=-0.093,P<0.05)。结论 血清FIB、uPAR成高表达水平的SLE患者疾病活动度更易加重,血清Ang2成低表达水平的SLE患者疾病活动度加重风险更高。Objective This paper aims to investigate the correlation between the serum levels of fibrinogen(FIB),angiopoietin 2(Ang2),and urokinase-type plasminogen activator receptor(uPAR) and disease activity in patients with systemic lupus erythematosus(SLE).Methods A total of 65 SLE patients admitted to the hospital from February 2020 to June 2022 were selected.According to the SLE disease activity index(SLEDAI) score,the SLE patients were divided into moderate and severe group(SLEDAI≥10 points)(n=34) and mild Group(SLEDAI<10 points)(n=31).The clinical data of the two groups was collected,and the correlation between the serum levels of FIB,Ang2 and uPAR and the disease activity of SLE patients was analyzed.Results Among the 65 SLE patients,the incidence of moderate to severe SLE disease activity was 52.31%,and the incidence of mild SLE disease activity was 47.69%.Univariate analysis showed that there was statistical significance on the differences in renal damage,serum FIB,Ang2 and uPAR levels between the two groups(P<0.05).Logistic regression analysis showed that the increase of serum FIB and uPAR levels and the decrease of Ang2 level were independent risk factors for the aggravation of disease activity in SLE patients(P<0.05).Spearman analysis showed that serum FIB and uPAR levels were positively correlated with SLEDAI(r=0.382,r=0.748,P<0.05),and serum Ang2 level was negatively correlated with SLEDAI(r=-0.093,P<0.05).Conclusion SLE patients with high serum levels of FIB and uPAR are more likely to increase their disease activity,while SLE patients with low serum Ang2 level is more likely to increase their disease activity.
