消脂降浊胶囊联合二甲双胍治疗肥胖2型糖尿病疗效观察

摘　　要：目的观察消脂降浊胶囊联合二甲双胍治疗肥胖2型糖尿病痰热郁阻证的疗效及对脂代谢的影响。方法选取肥胖2型糖尿病痰热郁阻型患者60例,随机分为对照组和观察组,各30例。对照组给予二甲双胍缓释片1.0g,2次/d口服;观察组在对照组基础上给予消脂降浊胶囊治疗。对比两组治疗前后中医证候积分、空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HBA1C)、胰岛素抵抗指数(HOMA-IR)、总胆固醇(CH)、甘油三酯(TG)、低密度胆固醇(LDL)、高密度胆固醇(HDL)、体质量、体质量指数(BMI)的变化。结果治疗后观察组临床总有效率为93.33%,对照组为76.67%,两组疗效比较,差异有统计学意义(P<0.05)。治疗后两组糖代谢指标、体质量指数均较治疗前有改善(P<0.05)。治疗后糖代谢指标改善方面观察组优于对照组(P<0.05);治疗后减重方面观察组优于对照组(P<0.01)。治疗后对照组中医证候积分及脂代谢指标均无明显改善,观察组中医证候积分较前明显下降,脂代谢指标TC、TG、LDL-C较治疗前均有明显下降,HDL-C较治疗前有明显升高,差异均有统计学意义(P<0.05),组间比较均优于对照组(P<0.05)。结论消脂降浊胶囊联合二甲双胍治疗肥胖2型糖尿病,在改善中医临床症状、糖脂代谢、减轻体重等方面均明显优于单纯二甲双胍治疗。Objective To observe the therapeutic effect of Xiaozhi Jiangzhuo capsule combined with metformin on obesity type 2 diabetes with phlegm heat stagnation syndrome and its effect on lipid metabolism.Methods A total of 60 obese type 2 diabetes patients with phlegm heat stagnation type were randomly divided into the control group and the observation group,with 30 cases in each group.The control group was given metformin sustained⁃release tablets 1.0g bid orally;The observation group was treated with Xiaozhi jiangzhuo capsule on the basis of the control group.The changes of traditional Chinese medicine(TCM)syndrome score,fasting blood glucose(FPG),2⁃hour postpran⁃dial blood glucose(2hPG),glycosylated hemoglobin(HBA1C),insulin resistance index(HOMA⁃IR),total cholesterol(CH),triglyceride(TG),low density cholesterol(LDL),high density cholesterol(HDL),body mass and body mass index(BMI)were compared before and after treatment.Results After treatment,the total clinical effective rate of the observation group was 93.33%,and that of the control group was 76.67%.The difference between the two groups was statistically significant(P<0.05).After treatment,the glucose metabolism index and body mass index of the two groups were improved compared with those before treatment,and there was statistical significance.After treatment,the observation group was better than the control group in the improvement of glucose metabolism index,with statistical significance(P<0.05);After treatment,the weight loss of the observation group was better than that of the control group(P<0.01).After treatment,the TCM syndrome score and lipid metabolism index in the control group were not significantly improved.The TCM syndrome score in the observation group was significantly decreased compared with that before treatment,the lipid metabolism index TC,TG,LDL⁃C were significantly decreased compared with that before treatment,and the HDL⁃C was significantly increased compared with that before treatment,the difference was statistically sig⁃nifican
